is a combined synopsis/solicitation for commercial items prepared in
accordance with the format in FAR subpart 12.6 as supplemented with
additional information provided in this notice. This announcement
constitutes the only solicitation: A quote is being requested and a
written solicitation will not be issued. Solicitation number
F6550100Q0242 is issued as a Request for Quotation (RFQ). The
incorporated document provisions and clauses are those in effect
through Federal Acquisition Circular 97-17. The Standard Industrial
Classification Code is 8744 with a Small Business size standard of $5.0
million. Contractor shall provide all parts, labor, tools, equipment,
supervision, personnel, materials and transportation necessary to
fulfill the requirements of the Program Coordinator of the Family
Services Program (FSP) in accordance with (IAW) the Performance Work
Statement (PWS). Delivery terms shall be FOB destination with service
to take place at Bldg 5188, 8th St, Elmendorf AFB, Alaska 99506.
Performance period is approximately 1 August 2000 through 30 Sep 2000.
The following provisions and clauses are incorporated and shall remain
in full force in any resultant purchase order; FAR Part 52.212-1;
Instructions to Offerors, Commercial Items is incorporated by
reference. FAR 52.212-2, Evaluation _ Commercial Items the blanks are
completed as follows: (a) the award will be made based on price and
price related factors, N/A. The government reserves the right to make
award on the initial quote received without discussions. Each offeror
shall include a completed copy of the provision at FAR 52.212-3;
Offeror Representations and Certifications- Commercial Items with its
quote. FAR Part 52.212-4; Contract Terms and Conditions- Commercial
Items, is incorporated by reference and applies to this acquisition.

Registration. Each offeror shall include a completed copy of the
provision at DFARS 252.212-7000 Offeror Representations and
Certifications _ Commercial Items and with its quote; 52.228-5,
Insurance_Work on a Government Installation; 52.245-4; Government
Furnished Property (Short Form). Incorporate DFARS 252.212-7001;
Contract Terms and Conditions Required to Implement Status or Executive
Order Applicable to Defense Acquisitions of Commercial Items. However,
for paragraph (b) only the following clauses apply, 252.225-7007; Buy
American Act_Balance of Payments Program, 252.225-7012; Preference for
Certain Domestic commodities, 252.227-7015; Technical Data_Commercial
Items, 252.227-7037; Validation of Restrict Markings on Technical Data,
252.243-7002; Requests for Equitable Adjustment, and 252.247-7024;
Notification of Transportation of Supplies by Sea. Acceptance of the
warranty does not waive the government_s right with regard to the other
terms and conditions of the contract. In the event of a conflict, the
terms and conditions of the contract shall take precedence over the
warranty. A firm-fixed price purchase order will be issued under the
Simplified Acquisition Procedures. While on the Internet, please
complete the Central Contractor Registration (CCR), as this is a
requirement prior to award of any contract. Additional information on
registration is available at 1-888-227-2423. DESCRIPTION OF SERVICES:
1.1 SCOPE OF WORK. The contractor shall provide all personnel,
equipment, tools, materials, vehicles, supervision, and other items and
services necessary to fulfill the requirements of the Program
Coordinator of the Family Services Program (FSP). This includes the
Loan Closet and Layette Program as defined in this Performance Work
Statement (PWS), except as specified in paragraph 3.0 as government
furnished property and services. Services shall be performed at 5188,
8th St., Elmendorf AFB Alaska.: 1.2 SPECIFIC DUTIES. The contractor
shall be responsible for the following requirements:1.2.1 Maintain
financial records for budget purposes. Budget information will be
reported to the Relocation Assistance Specialist (RAS). Attend Services
squadron meetings with RAS. 1.2.2 Monitor the overall effectiveness of
the FSP Loan Closet Program and Layette Program and make
recommendations for improving services and resolving problems. 1.2.3
Recommend FSP volunteers to the RAS for special recognition within the
guidelines of the FSP volunteer recognition program. 1.2.4 Document
volunteer service hours using the appropriate forms and create/maintain
a volunteer work schedule. 1.2.5 Ensure childcare procedures are
explained to those volunteers needing child care in order to work in
the Family Services Program Loan Closet and during special events.
Coordinate childcare for volunteers through the Elmendorf AFB Child
Development Center. 1.2.6 Maintain a current inventory of items for the
Loan Closet Program. Advises the RAS of the status of inventory in the
FSP program. Remove non-serviceable items and coordinate the
replacement/repair through the RAS. 1.2.7 Purchase supplies and items
for the Loan Closet Program. 1.2.8 Maintain copies of position
descriptions for volunteer positions within the FSP, review them
quarterly and recommend changes asneeded. Explain applicable job
descriptions to new volunteers. 1.2.9 Implement policy and procedures
for the Loan Closet and Layette Programs to insure client eligibility
and accountability for loans. Researches delinquent loan closet
contracts. 1.2.10 Provide information related to the completion and
submission of the annual United Way, Combined Federal Campaign (CFC)
Eligibility Application for eligibility consideration in the annual CFC
campaign. 1.2.11 Prepare all correspondence for the effective
administration of the FSP. 1.2.12 Assist with the Elmendorf AFB
Newcomer_s Tour. 1.2.13 Assist with the Annual Volunteer Recognition
Reception. 1.2.14 Responsible for the security of files, records,
providing reports, and information. 1.2.15 Maintain records to provide
monthly reports comprised of user statistical data using appropriate
Air Force forms on a date determined by the Quality Assurance (QA)
personnel. Files will be maintained in accordance with established file
plans. 1.2.16 Responsible for the orderliness and cleanliness of the
areas being utilized. These areas shall be kept clean and neat, free of
fire and safety hazards, and unsanitary conditions. 1.2.17 Maintain
telephone logs for all long distance calls by date, party called, phone
number, and purpose of the call. 1.2.18 Responsible for all the
carpeted areas shall be vacuumed weekly; the outside hallway shall be
swept when needed, but at least weekly; snow shall be removed from the
sidewalk in front of the entrance of the building daily and trash will
be collected and disposed of daily in the dumpster. The stairwell
leading to the loan closet shall be kept clear of debris. 1.2.19 Key
Control. The contractor shall establish and implement a method of
ensuring that the keys issued by the government are not lost or
misplaced and are not used by unauthorized personnel. The contractor
shall immediately report to the Quality Assurance (QA) personnel or
Contracting Officer any occurrence of loss, or unauthorized duplication
of keys. In the event keys are lost or duplicated, the contractor may
be required to re-key or replace the affected lock or locks without
cost to the government. 1.3 QUALIFICATIONS. The contractor shall have
knowledge of and experience in administering a loan closet program; a
layette program; recruiting, training, supervising volunteers;
recognition programs; and budget preparation and management to perform
the requirements of the PWS. The contractor shall have the following
qualifications: 1.3.1 A minimum of one-year experience as a paid or
unpaid employee in a similar type of program as stated in this
Performance Work Statement. 1.3.2 Knowledge of general office
procedures such as basic filing practices, record keeping, and proper
telephone courtesy. 1.3.3 Knowledge and ability to use computer
software such as, MS Word and Excel. 1.3.4 Excellent customer service
skills. 1.3.5 Maintain the highest standards of ethics and project a
professional image at all times. 1.4 The contractor shall submit the
following with their proposal: 1.4.1 A resume showing they meet the
outlined requirements. 1.4.2 Three current letters of reference with
the name, address, and current phone number of the individual writing
the recommendation. 2.0 RESTRICTIONS. 2.1 The contractor is prohibited
from receiving any monies or other benefits from any source as a
direct or indirect result of the services provided to FSP or relocation
clients. 2.2 The contractor shall distinguish clearly in public between
their statements and actions as an individual and as a representative
of the Family Support Center. 3.0 GOVERNMENT FURNISHED PROPERTY AND
SUPPORT. 3.1The government will provide the following facilities,
equipment, supplies, and support at no cost. The contractor shall be
responsible for safeguarding all government furnished property. 3.1.1
Workspace is located at 5188 8th St., Elmendorf AFB. 3.1.2 Reasonable
amounts of water, electricity, and other valuable utilities will be
furnished while performing work under this contract. The contractor is
expected to exercise conservation of base utilities. If the contractor
is found to be wasting base utilities, the contractor shall be required
to meter all utilities being used and shall be billed for utilities.
3.1.3 Provide assistance with the completion of and use of publications
directly related to the Family Services programs. Provide supplies,
forms, publications, and reference materials from government sources
directly related to the Family Services. 3.1.4 Telephones for official
use only. 3.1.5 Provide access to computers, printers, hardware,
software, and other materials and equipment necessary to maintain and
operate both stand alone computers and those linked to the base server.
3.1.6 Provide access to a copy machine for reproduction of materials
directly related to Family Services. The contractor shall comply with
applicable policies/regulations concerning the quantities and types of
material reproduced. 3.1.7 The government will provide equipment and
supplies necessary to perform snow removal, sweeping and vacuuming of
carpeted areas. 4.0 GENERAL INFORMATION 4.1Hours of Operation: 4.1.1
The contractor shall perform the services required under this PWS
during the following hours: Monday/Wednesday/Friday: 0900 hours _ 1500
hours; Tuesday/Thursday: 0900 hours _ 1300 hours. Monday, Wednesday,
and Friday the Loan Closet and Layette Programs are open for business.
Tuesday and Thursday shall be used to perform administrative tasks,
general cleaning of facilities, inventory and making purchases for the
loan closet. The Loan Closet and Layette Programs are not open for
business on Tuesday and Thursday. 4.1.2 The contractor shall provide
coverage during the above stated hours of operation to ensure the loan
closet and layette program is manned and operated without interruption
due to personal illness or other unforeseeable absences. 4.1.3
Recognized federal holidays include New Year_s Day, January 1; Birthday
of Martin Luther King Jr., the third Monday in January; Washington_s
Birthday, the third Monday in February; Memorial Day, the last Monday
in May; Independence Day, July 4; Labor Day, the first Monday in
September; Columbus Day, the second Monday in October; Veteran_s Day,
November 11; Thanksgiving Day, the fourth Thursday in November; and
Christmas Day, December 25. Please submit signed and dated offer to 3
