Contract base period is one calendar year from date award with two (2) one year options for renewal. The contractor is required to provide consultation, treatment, and surgery to include hospital services inclusive of all fees. Such fees include but not limited to: Professional fees, the use of cardiac catherization lab and /or operating room along with supplies, recovery room services, cardic-vascular perfusion, implantable hardware, central services, per diem room rates for the length of stay specified by the contractor, pharmacy, clinical laboratory blood bank, respiratory therapy, ECG/ Cardiographics, radiology, surgical pathology, echocardiography and graphics. Physicians who provide services must be board certified in this specialty area by the American Board of Medical Specialty. SIC: 8062. The issue date for this IFP is on or about July 18, 2000.
