This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR Subpart 12.6 and 13.5 as supplemented, with the additional information included in this notice. This announcement constitutes the only solicitation; This is a Request for Quote and a written solicitation will not be issued. Solicitation number RFQ 667-35-00; award will be made using the simplified acquisition procedures. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-18. This acquisition is unrestricted. The standard industrial classification (SIC) 7361. Authority: This procurement is executed under Enhanced Sharing Authority, Public Law 104-262, Title 38 U.S.C. 8153. THE SCHEDULE 1. Furnish 1 full-time equivalent (FTE)Radiologist to perform the requirements of this contract Est, 2080 Hr_______; Est Overtime Rate 100 Hr________; Est On Call/Standby Week Nights, 1 Day _________; Est On Call/Standby,1 EA_______. An FTE is defined as a contractors employee providing service 8 hours per day, 40 hours per week, 52 weeks per year. The above prices are all inclusive to include travel, lodging, car rental, malpractice and all related expenses with providing. 1. REQUIREMENT: The purpose of this contract is to provide Radiology services on an outpatient and inpatient basis, at the VAMC for a period of one year commencing on July 31, 2000 with the option to extend for one additional one year period in 30 day increments. a. The Radiologist is expected to meet as required Monday through Friday. b. Callback rate is defined as a one time charge, once called in, regardless how long the Radiologist is required to perform during a call. c. Callback or standby services to be provided by the contractor for after hours, weekends and holidays. This duty will be shared by the permanent staff of the VAMC. d. Therefore, billing for after hours services shall be those as specified in the schedule plus the established callback rate, once called in. e. The Radiologist will provide Radiology care and services for the following services: f. Routine (chest, skull, spines, abdomen, extremities, etc., IVP's (including tomograms), Retrograde/cystogram, Fluoroscopy, CAT scan, Ultrasound (organ), Carotid ultrasound and Doppler flow analysis, Venous Doppler analysis, Ultrasound prostate, Tomogram, Venogram, Myelograms (lumbar stick), Myelograms (cervical stick), Lumbar puncture, Percutaneous needle biops, Percutaneous needle aspiration, Percutaneous catheter placement and drainage, Cholangiogram, Percutaneous transhepatic cholangiogram, Cisternogram, Shuntogram, Arthrogram, Special Procedures (arteriogram, vena cavagram, arch flush, 4-vessel, aortogram, runoff, etc.) Subselective injections, Greenfield filter placement, Angioplasty, Consultation, Axillary sticks for arteriograms, Multiple sticks for arteriograms, Callback, Embolization of blood vessels, Percutaneous nephrostomy tube placements, Intra-arterial pressure gradient determinations, Enteroclysis, MRI, and MRA, preoperative and postoperative care, and outpatient clinic evaluation at the VAMC. g. The services to be performed by the contractor will be performed within the VAMC policies and procedures and the regulations of the medical staff by-laws of the VAMC. h. The services to be performed by the contractor will be under the direction of Chief Radiology Service (114) or his/her designee. i. The physicians under this contract must be credentialed and privileged at the VAMC, Shreveport, LA to perform Radiology Services. j. The VAMC will provide all medical records for the patients served by this clinic. All documentation will be made by the contractor in the medical record at the time of each visit in accordance with JCAHO Standards. k. All films that are matched must be read and dictated on the same day, if possible, but absolutely within one (1) workday. All films will be read and dictated on the VA Medical Center premises. l. Coordination of any surgeries, procedures or both, tobe performed at the VAMC will be provided by the contractor. m. A sign-in/out log will be set up for the contractor to sign in and out. The VAMC will pay for Services actually performed on site per this log. The contractor will be responsible for sick leave, vacation, holiday, and other benefits. m. Contract radiologists shall furnish their own communication system (pager). n. The Contractor will meet all appropriate JCAHO requirements regarding quality assurance such as maintaining current credentials and clinical privileges, peer review, participating in criteria development and data analysis, and providing guidance in meeting requirements of this contract. 2. TERMS OF CONTRACT: This contract is effective for one year with the option to extend for an additional one year in monthly increments. 3. QUALIFICATIONS: Personnel under this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia and Board Certified by the American Board of Radiology or Board eligible. Board eligible means Athe eligibility shall not be more than 4 years from the date the physician became eligible. The qualifications of such personnel shall also be subject to review by the VAMC Chief of Staff and approval by the VAMC Director. 4. WORK HOURS: a. Normal work hours are define as 8:00 a.m. through 4:30 p.m., Monday through Friday. b. When assigned, the contractor will be required to perform services on a national holiday or outside of normal working hours. When assigned, the contractor shall be available to perform services on a callback basis and shall respond within 30 minutes of the call. c. National Holidays: The ten (10) holidays observed by the Federal Government, i.e., New Years Day, President's Day, Martin Luther King Jr.'s Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. d. Off duty hours will be those of non-scheduled work days between the contractor and the Medical Center. 5. PERSONNEL POLICY: The contractor shall be responsible for providing: -- Health examinations. -- Worker's compensation. -- Professional liability insurance. -- Income tax withholdings. -- Social security payments. The parties agree that the contractor shall not be considered a VAMC employee for any purpose. 6. CONTRACT PERFORMANCE: a. The contractor will be monitored through a variety of mechanisms, including but not limited to: -- Clinic appointment lists showing patients seen. -- sign-in/out log. -- Chart documentation of patient diagnosis, treatment and or follow-up procedure. -- QA monitors designed to asses the quality of care provided in the clinic. -- Log books to document Radiology procedures done during the clinic. b. The Administrative Officer for Radiology (114) will be responsible for monitoring the quality of services provided under this contract. 7. CLAUSES The provisions at FAR 52.212-1, Instructions of Offeror -- Commercial Items (Mar 2000) applies to this acquisition. There is not an addenda to these provisions. The clauses at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (May 1999) applies to this acquisition. There is an addenda to this clause.
