This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation number F49642-00-T-0068 is issued as a request for quotation (RFQ) and will result in fixed-price line items with period of performance being completed 30 days after award. Incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 97-18. This acquisition is set aside for HUB Zone certified contractors. The SIC Code is 1752 and business size standard is $7 million. Wage Determination No. 94-2103, Rev 21, dated 9 Jun 00 can be located at www.ceals.usace.army.mil/netahtml/servcmg/html. In order to be considered responsive, offerors must certify in writing that their proposal meets all requirements set forth in this solicitation, attachment included. Schedule B. Non-personal Services: The contractor shall provide all plant, labor, equipment, tools, materials, transportation, supervision, and other items and services necessary to remove existing carpet, provide, deliver and install new carpet tiles and sheet vinyl. There are two locations that the contractor will be working, the first location is Building 17 and second location is building 1300, both are on Bolling Air Force Base, DC. The contractor shall perform all operations in accordance with industry standards and the attached statement of work. Description of Requirement/ Statement of Work (SOW). 1. SCOPE OF WORK: The contractor shall provide all plant, labor, equipment, tools, materials, transportation, supervision, and other items and services necessary to remove existing carpet, provide, deliver and install new carpet tiles, sheet vinyl and cove base. There are two locations that the contractor will be working, the first location is Building 17 and second location is building 1300, both are on Bolling Air Force Base, DC. The contractor shall perform all operations in accordance with industry standards and the attached statement of work. Following are estimated quantities for determining possible workloads: Building 17 Remove approximately 1401 square yards of glued down carpet. Install approximately 1401 square yards of carpet tiles. Remove and install approximately 1888 linear feet of cove base. Install approximately 100 linear feet reducers and transition strips. Building 1300 Remove approximately 782 square yards of glued down carpet. Install approximately 2700 square feet of sheet vinyl with heat welded seams (first floor hallways only). Install approximately 482 square yards of carpet tiles. Remove and Install approximately 1600 linear feet of cove base. Move furniture as necessary. Install approximately 350 linear feet reducers or transition strips. All measurements and quantities are estimated. The contractor is responsible for verifying all quantities and measurements for all requirements associated with the project. 2. CONTRACTOR REQUIREMENTS: The contractor shall be experienced and competent in the installation of sheet vinyl flooring using heat welded seams. The contractor shall also be thoroughly experienced in carpet installation with installers having no less than two years experience in installation of carpet or flooring projects similar to that required in this Statement of Work. 2.1. The contractor shall confirm all given information and advise the contracting officer, prior to bid, of any conflict that will affect their work. 2.1.1 Schedule and execute work to prevent excessive traffic on completed sections of flooring. It will be necessary to establish and maintain adequately ventilation of the work area without causing entry of contaminants to other parts of the building. Individuals who are sensitive to fumes and odors shall be advised to avoid the area while work is in progress. 2.1.2. The contractor shall be thoroughly experienced and upon request be able to provide evidence of having successful experience installing the required materials. 2.2. SECURITY REQUIREMENTS: The contractor shall submit names of all personnel assigned on site prior to start of work for base passes and security clearance requirements. 2.3. SUPERINTENDENCE BY CONTRACTOR: The contractor shall provide adequate supervision of his employees at all times to ensure complete and satisfactory performance of all work in accordance with the terms of the contract. 3. GENERAL REQUIREMENTS: The contractor shall remove the existing carpet and cove base and provide and install the following or equal materials: CARPET TILE Manufacturer: Collins & Aikman Style: Martinique Color: 38409 Caribbean SHEET VINYL Manufacturer: Armstrong Style: Medintech, Commercial backed felt with matching welding rods. Size: six feet wide, up to 90 feet long Gauge: 0.080 Color: 91477 Liatris COVE BASE Type: Vinyl Color: Medium gray Thickness: 1/8_ Height: 4_ 3.1. Product delivery, storage and handling. The contractor shall deliver materials in good condition to the job site in manufacture's original unopened containers with label information clearly marked. 4. PRE-INSTALLATION REQUIREMENTS: Before installation services are started, the contractor shall inspect the work site and ascertain all information necessary to deliver effective performance of services. The contractor shall notify the Contracting Officer of any conditions that prevent the suitable completion of these services. 4.1. Contractor shall submit manufacturer_s standard sample sets in form of pieces cut from actual carpet and cove base to be installed. 4.2 The performance period shall be 30 days after award. Work dates shall be coordinated with government representative for final approval, prior to start of work for. 5. STANDARD FURNITURE MOVING: If it is necessary for the contractor to install carpet under module furniture, the contractor shall be required to use manufacturer recommended methods so as not to damage or void any warranties. 5.1. The contractor shall be responsible for moving and protecting non-sensitive equipment and furniture in the work area and replacing it in the same location when work is completed. Highly sensitive equipment such as computers and backup tapes shall be the responsibility of the using agency. 6. REMOVING EXISTING CARPET: In removing cemented rubber or vinyl backed carpeting, the contractor may cut the carpet into strips or, if necessary, remove with an electric floor striper. Remove residue adhesives or cover with approved under layment as recommended by the flooring manufacturer. 6.1. Contractor is responsible for removing and replacing cove base. New 4_ vinyl cove base color shall be matched with existing. 6.2. All carpet removed shall be disposed of by the contractor at an off-base site, meeting all local, County, State and Federal Regulations. 7. INSTALLATION: Carpet installation shall be by direct glue procedures in accordance with manufacturer's recommendations. When installation is accomplished, all carpet shall fit snugly to the floor and shall have no bulges, wrinkles, bubbles, open seams or other irregularities affecting the appearance and serviceability. 7.1 Sheet vinyl shall be installed in strict accordance with manufacturers written instructions with a minimum of seams using the commercial backed installation system with adhesive and heat-welded seams as recommended by the manufacturer. 7.2. Floors shall be clean dry, smooth, and flat. remove all ridges, and other irregularities. Fill all cracks, holes and depressions with compounds recommended by the manufacturer. All adhesives used for installation of flooring and base materials shall be those recommended by the flooring manufacturer. Adhesive shall be applied using the full spread quick release method. 7.3. Extend flooring materials into toe spaces, door reveals, closets and similar openings. 7.4. Install wall base in strict accordance with manufacturer's instructions. Install shall be competent in wallbase installation system. 8. CLEANING. The contractor shall remove surplus materials and debris daily, upon completion of work and leave the work area in a clean, neat and condition. 8.1. Contractor shall vacuum or mop areas after carpet and vinyl installation. Remove all cuttings and scrap materials. Glue spots and residue shall be cleaned from the carpet and vinyl and wallbase, using manufacturer's suggested cleaning products. 9. COORIDINATION. The point of contact for this project is the Quality Assurance Evaluator, Mr. Melvin Holston at (202) 767-1090. 10. HOURS OF OPERATIONS. Hours of operation shall be from 4:00 PM to 12:00 AM Monday through Friday. 11. QUALITY ASSURANCE: According to the Inspection of Services clause, the government will evaluate the contractor_s performance using customer complaint and periodic methods of surveillance. 12. AVAILIBILITY OF GOVERNMENT SERVICES. Water, electrical power for construction purposes and lighting is available at the site and will be made available to the contractor. 13. SAFETY. The contractor shall be responsible for all means and methods they relate to safety and shall comply with all local, state and federal requirement that safety related. All personnel shall instructed daily to be mindful of the full time requirement to maintain a safe environment for the facilities occupants. 14. WARRANTY. The contractor shall provide a written warranty agreeing to repair or replace flooring in the event materials or workmanship fails within the specified warranty period. 15. INVOICES: The contractor shall submit itemized invoices reflecting all intervening service calls including labor and parts and scheduled preventative maintenance on a monthly basis. Invoices shall show the contract number, the name of the equipment, building number, date of service. Invoices shall be submitted in arrears in four (4) copies to Service Contract Management, 11 CES/CEOEC, 370 Brookley Avenue, Bolling AFB, DC 20332-6000. No payments will be made until an invoice, with properly acknowledged documentation is received and approved by the Contracting Officer or the Government representative. FAR Clauses. FAR 52.212-1, Instructions to Offerors Commercial Items is hereby incorporated by reference. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation and is determined most advantageous to the Government. Award will be made on an all or none basis. Offerors shall complete copy of the provision at FAR 52.212-3, Offer Representation and Certifications _ Commercial Items and DFAR 252.212-7000, Offer Representations and Certification _ Commercial Items with its offer.
