Q -- MEDICAL OFFICER OF THE DAY SOL RFQ 564-2-01 DUE 081700 POC Eleanor Dalmut (501) 444-5027 This procurement is hereby issued for commercial items to be used by the VA and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12, Acquisition of Commercial Items, Subpart 12.6 and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. Simplified acquisition procedures apply and the VA will conduct the procurement according to a combination of Parts 12 and 13. Solicitations are being requested under Public Law 104-262 The Veterans Health Care Eligibility Reform Act of 1996 and Title 38 United States Code Section 8153, with a Commercial Items format of terms, conditions, and provisions/clauses. This procurement is being conducted under Federal Acquisition Regulation (FAR) Subpart 13.5 test program. This is 100% Set-Aside for Small Business. Contractor shall provide the services of a Board Certified Internal Medical or Family Practice physician to perform Medical Officer of the Day duties. The services shall be provided one day a week, weekends, holidays and scheduled leave for the VA Medical Center, Fayetteville, AR in accordance with the terms and conditions of the contract. Contract period is 11-1-00 through 10-31-01 with four one-year options to extend the term of the contract. Physicians must be certified, licensed, and attain and hold an in-force Professional Malpractice Insurance in an amount not less than one million dollars ($1,000,000.00) during the contractual period along with other requirements as indicated in the solicitation package.
