AFB Clinic, MA is soliciting for a Nurse Health Educator (NHE)to
provide all labor, health and wellness education, teaching and
training, consultations and reports to Government beneficiaries IAW the
Statement of Work (SOW). This procurement shall consist of one (1)
basic year and four (4) option years. This contract is a personal
services contract which is intended to create an employer relationship
between the Government and the individual. This is authorized by the
Federal Acquisition Regulation (FAR) Part 37, DFARS 237-104 and 237-106
and the Department of Defense Instruction (DODI) 6025.5 authorized by
10 U.S.C. 1091. Exercise of each option year will be completed on an
independent, year to year basis. Evaluation will be based on contractor
performance, the requirements of the Hanscom Clinic and the
availability of funds. Upon being hired, the applicant will be
considered a self-employed, independent contractor and will be
responsible for paying all required state and federal taxes, social
security, health insurance, etc. The offeror's package should include
a complete resume and all documentation of required licensing and
certification. The basic year and all four (4) option years should be
responded to separately, with inflation taken into consideration. All
offers should be in the form of a Price Per Hour. The contractor will
be paid only for hours worked. (No paid lunch hour, vacation time, sick
leave, etc.) The contractor will be fully covered under Hanscom Clinic
and will not require any additional personal liability insurance.
Award will be made on a best-value basis with qualification and
experience of th e applicant being the most important factor and price
being a secondary factor. All offers are due no later than 31 July
