Burkett, Contracting Officer (540) 983-1084 Solicitation was originally advertisied with an issue date of o/a August 1, 2000 and proposal due date of September 1, 2000. Due to critical need for services this is being changed to a combined sysopsis/solicitation for commerical items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under simplified acquisition procedures in accordance with FAR Part 13. This announcement constitutes a formal Request for Proposal (RFP) and a written solicitation WILL NOT be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 97-18. RFP 658-50-00 shall be referenced on all proposals. The Standard Industrial Classification (SIC) for this procurement is 8099 and the small business size-standard is gross annual receipts NTE $5.0 million. This procurement is unrestricted. The VA Medical Center, 1970 Roanoke Boulevard, Salem, Virginia 24153, has an emergent need for a Locum Tenes TPA Certified Optometrist to provide services 40 hours per week, Monday through Friday, 8:00a.m. to 4:30p.m., excluding Holidays. Optometrist shall not be required, except in case of emergency, to furnish such services on a National Holiday or during off-duty hours. VAMC Salem, Virginia intends to solicit proposals and award a firm, fixed price contract under the authority of Title 38 USC Section 8153. The contract is for a six (6) month period but may be extended for an additional six (6) months. Optometrist shall be a citizen of the United States and shall hold a Doctor of Optometry Degree from an approved school of college of Optomery. Optometrist shall hold a valid and unristricted license to practice Optometry in a State, Territory, or Commenwealth of the United States or the Dristric of Columbia and shall be proficient in the English language, both spoken and written. Duties shall be described herein: (1) Provide detailed diagnostic, therapeutic and reabilitative rehabilitative procedures necessary for the management of a) refractive anomalies, b) problems of binocular vision, ocular motility, accommodation or visual perception, c) other complex visual and ocular anomalies including aphakia, keratoconus, aniseikonia, etc. (2) Perform procedures necessary for the diagnosis and management of ocular disease and ocular manifestations of systemic disease, including the use of pharmaceutical agents. Licensure shall include authorization to use therapeutic agents. (3) Make appropriate use of laboratory or radiologic testing in the diagnosis and management of ocular disorders. (4) Perform other special procedures within the scope of practice of Optometry including, but not limited to, a) corneal foreign body removal, b) lacrimal dilation and irrigation, c) photography of the ocular fundus or anterior segment, d) nasolacrimal occlusion via temporary or permanent punctal plugs. (5) Arrange referral to or consultation with other disciplines within the Medical Center when indicated. (6) Participate in the education of Optometric students and residents by serving as a clinical preceptor or by other means. (7) Participate in visual or ocular research or other scholarly pursuits. Offerors shall submit the following with their proposals: pricing schedule; curriculum vitae, medical license (from a State, Territory, or Commonwealth of the United States or the District of Columbia), evidence of TPA Certification, and evidence of Indemnification and Medical Liability Insurance. Offerors may propose more than one (1) candidate. The Contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the Contractor shall provide the following; Worker's Compensation Insurance, Professional Liability insurance Income Tax Withholding, Social Security Payments, and Health Examinations. Contractor personnel shall provide proof of the following tests within five (5) calendar days after award and prior to the first duty shift. Tests shall be current within the past year. Tuberculosis Testing: All Contractor personnel shall provide proof of a negative reaction to Purified Protein Derivative (PPD) testing. A negative chest radiographic report for active tuberculosis shall be provided in cases of positive PPD results. The PPD test shall be repeated annually. Rubella Testing: All Contractor personnel shall provide proof of immunization for measles, mumps, rubella or a rubella titer of 1.8 or greater. If the titer is less than 1.8, a rubella immunization must be administered with follow-up documentation to the Contracting Officer's Technical Representative (COTR). OSHA Regulation Concerning Occupational Exposure to Bloodborne Pathogens: The Contractor shall provide a generic self study training module to its personnel; provide Hepatitis B vaccination series at no cost to its personnel who elect to receive it; maintain and distribute an exposure determination and control plan to its personnel; maintain required records; and ensure that proper follow-up evaluation is provided following an exposure incident. Va will notify the Contractor of any significant communicable disease exposures as appropriate. The Contractor's occupational health provider shall adhere to current CDC/HICPAC Guidelines for Infection Control in health care personnel (AJIC 1998; 26:289-354) for disease control. The contracting agency shall provide follow-up documentation of emploee's clearance to return to the workplace prior to their return. Offerors shall provide a completed set of provisions with their offer found at FAR 52.212-3. Offer Representation and Certifications-Commercial Items. Provisions and clauses found at 52.212-1, 52.212-4 and 52.212-5 apply. The following FAR and VAAR clauses and provisions are added by addendum 52.224-1 Privacy Act Notification; 52.224-2 Privacy Act; 852.237-7 Indemnification and Medical liability Insurance; 852.270-4 Commercial Advertising; and 852.271-70 Services Provided Beneficiaries. Offerors may find the FAR provisions and clauses on the Internet at www.arnet.gov/far/. Proposals shall be accepted immediately, but no later than Julu 28, 2000. Due to the critical need of these services, award may be made prior to July 28, 2000. An official authorized to bind the offeror's organization shall be required to sign all offers.
