Period of performance will be a base year with two (2) one (1) year options to renew (If all options are exercised on this contract it will be in effect until September 30, 2003). Offerors shall provide/perform the following services: (a) comprehensive vocational rehabilitation evaluation; (b) career guidance; (c) rehabilitation planning; (d) preparation of report of evaluation findings. Multiple awards are anticipated with a minimum of one contract for each geographical area, within Utah, noted in the Statement of Work. A Firm Fixed Price contract is anticipated.
