The Southern Arizona VA Health Care System (SAVAHCS), Tucson, AZ requires semi-annual surveys to assess patient satisfaction and opportunities to improve delivery of patient care. Surveys shall be random samplings of patients from a variety of clinical specialty areas of the facility, e.g., general surgery, cardiology, dermatology, etc. Questionnaire types shall include adult inpatient, adult inpatient rehabilitation, adult office visit, and adult home care. It is expected approximately 2540 surveys will be sent out for each semi-annual cycle prorated across the various clinical specialty areas. The contractor shall provide a complete turnkey survey system to include survey management, survey administration, postage, analysis, and reporting. The contract is expected to be for a base year with 4 one-year option renewals. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Request for Quotations (RFQ) are being solicited and a written solicitation will not be issued.
