Agency's (US EPA) Office of Research and Development (ORD), is seeking
a qualified contractor to provide the following items: (1) Lancer USA
Model 1600UP glassware Washing Machine with 4KW Drying, 4 Wash Levels
and 40 Microprocessor-Controlled Programs. Concealed Storage for
Detergent and Acid. LCD and Audible alarms and 6 Rotary Spray Arms.
Constructed with 304L Stanless Steel Throughout-Double wall, Insulated
chamber. Built-in Water Softener and Deionized Water Pump. Built-in
Acid Pump and Liquid Detergent Dispenser. Triple Filter System for
Protection of Circulation Pump, quantity of one each, (2) LCD 2.5
LancerClean Detergent (2.5 Gallon), quantity of two each. (3) LCA 2.5
LancerAcid (2.5 gallon), quantity of two each. FOB Destination is being
requested in which the final destination is ADA, Oklahoma. This is a
combined Request For Quotation (RFQ) and Synopsis. NO SEPARATE REQUEST
FOR QUOTATION WILL FOLLOW THS ANNOUNCEMENT. The following clauses are
applicable to this RFQ: EPAAR 1552.233-70, NOTICE OF FILING
REQUIREMENTS FOR AGENCY PROTESTS; FAR 52.204-2, TAXPAYER IDENTIFICATION
(JUN 97), FAR 52.213-4(A), 52.213(B)(1), 52.213(C), 52.213(D),
52.213(F), 52.213(G), 52.213(H), TERMS AND CONDITIONS -- SIMPLIFIED
ACQUISITIONS (OTHER THAN COMMERCIAL ITEMS) (FEB 98). COMPLIANCE WITH
VETERANS EMPLOYMENT REPORTING REQUIREMENTS (99-1)(FEB 1999) (a) The
Offeror represents that, if it is subject to the reporting requirements
of 38 U.S.C. 4212(d) (i.e. the VETS-100 report required by the Federal
Acquisition Regulation clause 52.222-37, Employment Reports on
Disabled Veterans and Veterans of the Vietnam Era), it has or has not
(please specify) submitted the most recent report required by 38 U.S.C.
4212(d). (b) An offeror who checks "has not" may not be awarded a
contract until the required reports are filed (31 U.S.C. 1354). If you
have not filed with the VETS-100 report, information can be obtained
from the DOL website as follows: Step 1 -- Go to http://www.dol.gov.
Step 2—Click on DOL Agencies. Step 3 -- Click on Veteran
Employment and Training Services. Step 4 -- click on -- VETS-100. The
award will be made to the lowest priced vendor that meets all of the
