The Fleet and Industrial Supply Center Norfolk, Washington Detachment, has a requirement for the National Naval Medical Center-Radiology Department. FISCNORDETWASH to purchase on-call and preventive maintenance for Trionix Dual Head Gamma Camera, Item 0001, Trionix Biad Spect Gamma Camera Model #BIADI S/N E-4 (B102) 01011295 with workstation S/N 147M1788 01-014652, Item 0002, BIAD II S/N SR 154(BIAD II) 01-0133575. These items shall require 24 hours, seven days a week emergency service with preventive maintenance. The contract will be for one base year and four one year option years. The contractor will be required to provife telephonivally respond within 2 hours aftter receipt of notification of equipment failure and shall provide on site service within 24 hours. Preventive maintenance shall be performed every three months. Preventive maintenance includes complete examination of the equipment's functional conditions, including, but not limited tothe instrumentation of electronic, electromechanical, mechanical and chemical functions; adjustment, cleaning, calibration, lubrication, and replacement of worn out parts as necessary. All equipment shall be label with an inspection sticker after all preventive maintenance has been performed. (NOTE: All maintenance, repairs and service shall be in accordance with JCAH manufacture's National Fire Protection Act (FSD)). This will be a fixed priced contract for a base year plus four one year options periods. Options to Extend Services (AR 52.217-9 (AUG 1989) will be incorporated into this contract.
