Amendment 0003 to Request for Proposals PR-R3-00-10024 Operation and
Management of Environmental Protection Agency Region III EXCL Wellness
Center. This notice is the Amend!!!! The hour and date specified for
the receipt of offers is 3PM ET on Mon Aug 7, 2000. Offerors must
acknowledge receipt of this amend prior to 3 PM ET on Mon Aug 7 by one
of the following methods: a) by acknowledging receipt of this amend on
each copy of the offer submitted; or b) by separate letter or fax which
includes a reference to the RFP and amend numbers. Failure of your
acknowledgment to be received at the place designated for the receipt
of offers prior to 3PM ET on Mon Aug 7 may result in rejection of your
offer. If by virtue of this amend you desire to change an offer
already submitted, such change may be made by letter or fax, provided
each letter or fax makes reference to this RFP and amend, and is
received prior to 3PM ET on Mon Aug 7. Except as provided herein, all
terms and conditions of the RFP, as heretofore changed, remain
unchanged and in full force and effect. Any further amendments will be
announced in the Commerce Business Daily. The following are answers to
questions received: 54 revised)How many full time and part time staff
members does the management company currently employ. Does EPA and BoG
find that number of staff acceptable for quality service desired?
These questions must be submitted to the Govt IAW the Freedom of
Information Act. 55)What items are carried by the BoG Incentive Program
Store? Are items sold to EXCL Wellness Center members, to EPA employees
and/or others? Different items are purchased by the Bd of Governors for
each incentive program, such as sweatshirts, socks and shorts. Members
successfully participating in the incentive programs receive the items
at no cost. Depending on quantity purchased, any extra items are then
available for sale to the members only. When a piece of Bd Of Governors
exercise equipment is replaced, the replaced item, if not traded in,
may be made available for sale to members or donated to charity.
56)What are the sources of income for the Bd of Governors? What
expenses are used by this income? The sources of Bd of Governors income
are members dues set by the Board, class fees and Incentive Store
sales. The income is used for such things as the equipment maint
contract, incentive items, hygiene products (soap, shampoo, hair
dryers, equipment towels, locker room rugs), bottled water, newspapers,
magazines, music, televisions and other AV equipment, aerobic and
anaerobic exercise equipment, accessories, and classes. 57)Can we
submit addl info about our programming via an interactive website
demonstration? No. Offerors should not provide any information other
than required by the solicitation. See 52.212-2 and EP52.215-105.
