a combined synopsis/solicitation for commercial items prepared in
accordance with the format in Federal Acquisition Regulation (FAR) sub
part 12.6., as supplemented with additional information included in
this notice. This announcement constitutes the only solicitation; a
proposal is requested and a written solicitation will not be issued.
This combined synopsis/solicitation is issued as a Request for
Proposal. The Air Force Medical Logistics Office/Veterans Affairs
Special Services contemplates negotiation on a sole source basis with
Repro-Med Systems, Incorporated which is the sole manufacturer of the
Res-Q-Vac Hand Powered Medical Suction Device, Freedom 60 Syringe
Infusion System, and the Masterson Endometrial Biopsy Kit. These items
are handheld, lightweight, compact, portable, and manually powered.
Under the authority of FAR 6.302-1, Only One Responsible Source and No
Other supplies or Services Will Satisfy Agency Requirements, the
Government intends to solicit and negotiate with only onesource. The
resulting contract will be a Government Wide (CONUS and OCONUS) IDIQ
with a base period and 4 option years if exercised. Solicitation
documents and incorporated provisions and clauses are those in effect
through Federal Acquisition Circular 97-19. SIC is 5047 and small
business size is 500. Equipment shall be shipped FOB Destination
(within CONUS) within 30 calendar days after receipt of delivery order.
The following incorporated provisions and clauses are those in effect
through FAC 97-19 and are applicable to this solicitation: FAR
52.212-1, Instructions to Offerors Commercial Items: Addendum 52.212-1,
Paragraph (b)(5); Offerors shall provide an express warranty which at
a minimum shall be the same warranty terms, including offers of
extended warranties, offered to the general public; FAR 52.212-4,
Contract Terms and Conditions -- Commercial Items; FAR 52.212-5,
Contract Terms and Conditions Required to Implement Statutes or
Executive Orders -- Commercial Items; Offerors must include (1) a
completed copy of the provision at FAR 52.212-3, Offeror
Representations and Certifications -- Commercial Items, (2) Commercial
and Government Entity (CAGE) code, Contractor Establishment Code (DUNS
number), and Tax Identification number. All FAR provisions and clauses
may be reviewed and/or obtained from the General Service Internet
address http://www.gov/far. This notice of intent is not a request for
competitive proposals. A determination by the Government not to
compete this proposed contract, based upon responses to this notice, is
solely within the discretion of the Government. Information received
will be considered solely for the purpose of determining whether to
conduct a competitive procurement. All responses must be submitted no
later than August 18, 2000 at 2pm. Questions concerning this
