Division of Office of Public Health and Science, Office of Minority
Health through the Program Support Center, General Acquisitions Branch,
plans to award a 12 month sole source Simplified Acquisition to the
Connecticut Department of Public health. The anticipated date of award
is September 1, 2000. The purpose of this contract is to develop
strategies for a statewide health plan that will address the
elimination of race and ethnic health disparities in Mississippi. The
plan will be utilized to improve the health status of minorities.
Specifically, funds from this contract will be used to: (1) establish
a steering committee with public and private partners that participated
in the OMH Region IV sponsored Mississippi Conference on the
Elimination of Health Disparities held in March 2000; (2) convene nine
statewide public forums to identify/discuss barriers to health care
and to develop strategies to eliminate these barriers as part of the
overall health plan; and (3) create a sustainable network ofpartners
who will continue to assess the needs of their communities. The
Contractor shall provide the support necessary to carry out the
following tasks: 1. In collaboration with Region IV Minority Health
Consultant and Health Department Officials, identify public/private and
community-based participants from the March 2000 Mississippi Health
Disparities Conference to serve on a steering committee which will
provide input to the development of the statewide health plan. 2. In
collaboration with Health Department Officials, review and analyze
available minority health data that will be used to develop the health
plan. 3. In collaboration with the steering committee, prepare plan
for holding nine public forums (portions will be recorded).
Gather/organize materials for public forums and provide other
logistical support. 4. Compile recommendations/strategies from public
forums and prepare draft strategies/recommendations for the statewide
plan. 5. Submit draft strategies to steering committee for review
andapproval. Finalize draft based on the review. 6. Prepare written
quarterly status reports and a final report of the project THE PROPOSED
