VIM2958@BOP.GOV. This is a combined synopsis/solicitation for a
commercial item prepared in accordance with the format in FAR Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; proposals
are being requested and a written solicitation will not be issued. The
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 97-18. The provisions at
52.212-1, Instructions to Offerors-Commercial Items; 52.212-4, Contract
Terms and Conditions-Commercial Items; 52.212-5, Contract Terms and
Conditions Required to Implement Statutes or Executive
Orders-Commercial Items; and 52.237-7, Indemnification and Medical
Liability Insurance all apply to this acquisition. Offerors must
include a completed copy of the provisions at 52.212-3, Offeror
Representations and Certifications-Commercial Items with their bid in
order to submit a complete bid package. All provisions and clauses can
be accessed electronically at http://www.arnet.gov/far/. Offerors may
request a copy of 52.212-3 from the Contracting Officer, if needed.
This synopsis/solicitation is for Optometry services from a licensed
Optometrist. The contractor shall provide Optometry services to the
inmate population at the Federal Correctional Complex, Victorville, CA.
The duties will include eye health examinations, evaluations,
diagnosis, treatment and recommendations for certain opthalmological
conditions and the contractor will perform administrative functions to
include inventory of supplies. The contractor will also perform
refractions for use in fitting patients with glasses or contact lenses
following the guidelines outlined in the Health Services Department.
The contractor will maintain records in accordance with standard
community practice and the Bureau of Prisons guidelines. All supplies
and materials routinely required will be provided by FCC Victorville.
All records generated by this contract are and shall remain the
property of the Federal Bureau of Prisonsand require adherence to the
Privacy Act of 1974. The contractor will utilize proper technique and
proper managment of all equipment. The contractor will have a Doctorate
Degree in Optometry and be Board Certified or eligible for
certification in Optometry. The contractor will possess a current
license to practice Optometry in the state of California and the
contractor should have a minimum of four years experience as an
Optometrist. The contractor will possess current liability insurance as
outlined in 52.237-7 of the FAR. Proof of insurance, verification of
education, training and experience is required by the contractor and
these documents will be kept in the contract file. Quotations will be
based on 1 (one) session per week, with a session defined as four
hours. Sessions will be scheduled Monday thru Friday, the exact times
will be arranged by mutual agreement between the contractor and the
Health Systems Administrator. This is a contractual agreement and not
an appointment, the service does not constitute an employer/employee
relationship. Before contract award, the apparent low bidder will be
required to complete and pass the following security requirements:
National Crime Information Center (NCIC) check, DOJ-99(Name Check),
FD-258 (Fingerprint Check), Law Enforcement Agency Checks, Vouchering
of Employers, Resume/Personal Qualifications, OPM-329-A (Authority for
Release of Information), National Agency Check and Inquiries
(NACI)check (if applicable)and the contractor (or employee of
contractor performing services) is required to take a urinalysis test,
requiring a negative result. This solicitation is a Request for
Quotation, number RFQ-236-0017-0. This contract will be for Fiscal Year
2001, commencing on October 1, 2000 ending on September 30, 2001. The
Government will award a contract resulting from this solicitation to
the responsible offeror whose offer, conforming to the solicitation,
will be most advantageous to the Government. Quotations are due in our
office on August 29, 2000 by 4:00pm, Pacific Standard Time. Quotes
