The VA will provide the equipment (one Lorad M-IV Mammography System- upgradable to digital) and space located at 1700 Vegas Drive, Las Vegas, NV 89106. The Contractor will provide everything else including: a) Staffing -- Radiologic Technologist services, Interpreting Radiologist services, administrative support, and/or Medical Physicist duties; b) FDA application/accreditation; c) Compliance with MQSA -- Final Rule documentation; and d) FDA annual inspection and documentation. This is NOT A REQUEST FOR PROPOSALS, but a request for information. The purpose of this Request for Information is to obtain information on potential companies that have the capacity, experience, and other necessary resources to provide mammography services. All proprietary information should be so marked and the information will be fully protected to the extent allowed.
