synopsis/solicitation for commercial items prepared in accordance with
the format in FAR Subpart 12.6 as supplemented with additional
information included in this notice. This procurement is conducted
under simplified acquisition procedures in accordance with FAR Part 13.
This announcement constitutes a formal Request for Proposal (RFP) and
a written solicitation WILL NOT be issued. The solicitation document
and incorporated provisions and clauses are those in effect through
Federal Acquisition 97-18. RFP 658-53-00 should be referenced on all
proposals. The Standard Industrial Classification (SIC) for this
procurement is 8099 and the small business size-standard is gross
annual receipts NTE $5.0 million. This procurement is unrestricted. The
VA Medical Center, Salem, Virginia has an emergent need for a Locum
Tenens Physician Board Certified in Urological Surgery (candidate(s)
may be a Board Eligible Physician if within four (4) years of
postgraduate residency and/or internship training) to provide surgical,
administrative and clinical Urological services at the Veterans Affairs
Medical Center, 1970 Roanoke Boulevard, Salem, Virginia 24153.
Physician shall provide coverage Monday through Friday, 7:45 a.m. --
4:30 p.m., excluding Federal Holidays. Physician(s) must have Basic
Life Saving (BLS) certification. Physician shall not be required,
except in case of emergency, to furnish such services on a National
Holiday or during off-duty hours. Physician(s) shall perform the
following administrative and clinical services at the VAMC, Salem,
Virginia, which include the following: (1) Provide Pre-operative and
Post-operative Urology Services, including hospital rounds for Urology
patients. (2) Conduct weekly outpatient and inpatient clinics on
Monday, Tuesday, and Thursday each week (excluding Federal Holidays).
(3) Conduct surgical procedures on Monday, Wednesday, and Friday. (4)
Provide non-operative Urology hospital rounds for Urology patients. (5)
Authentication of Medical Records. (6) Critique Morbidity and Mortality
Information and Documentation. (7) Responsible for the dictation of
clinic and operation notes, and discharge summaries. (8) Contractor's
employee(s) shall provide on-call coverage on an as needed basis as
coordinated with Acting Chief, Surgical Care Service Line. (9) Clinic
Schedule shall be held on Monday (Ultrasound Biopsy Clinic, 12:30 p.m.
to 2:00 p.m. 5 Patients, and Urodynamics Clinic, 3:00 p.m. to 4:00
p.m. 2 Patients); Tuesday (Urology Consult Clinic, 8:00 a.m. to 10:00
a.m. 10 Patients, Ultrasound Biopsy Follow-up Clinic, 10:30 a.m. to
12:00 p.m. (Noon) -- 5 Patients, and Urology Follow-up Clinic, 1:00
p.m. to 3:00 p.m. -- 10 Patients), and Thursday (Urology Consult
Clinic, 8:00 a.m. to 10:00 a.m. 10 Patients, and Urology Follow-up
Clinic, 1:00 p.m. 3:00 p.m. -- 10 Patients). (10) Physician(s) shall
perform the following operating room surgical schedule: (1) Monday,
8:00 a.m. to 12:00 p.m. (Noon) (Cystoscopies Only)(5 to 7 Procedures),
Wednesday, 8:00 a.m. to 12:00 p.m. (Noon) (5 to 7 Procedures), and
Friday, 8:00 a.m. to 12:00 p.m. (Noon) (5 to 7 Procedures). (11)
Contractors physician shall be responsible for a variety of major and
minor surgical procedures, and some emergency surgical procedures
during other than normal working hours. Offerors shall submit the
following with their proposal: pricing schedule; Curriculum Vitae,
medical license (from a State, Territory, or Commonwealth of the United
States or the District of Columbia), evidence of board certification,
evidence of ACLS/BLS Certification and DEA certificate of proposed
physician(s); and evidence of company's Indemnification and Medical
Liability Insurance. Offerors may propose more than one candidate. The
Contractor shall be responsible for protecting the personnel
furnishing services under this contract. To carry out this
responsibility, the Contractor shall provide the following: Worker's
Compensation Insurance, Professional Liability Insurance, Health
Examinations (to include current TB Test results), Income Tax
Withholding, and Social Security Payments. Offerors shall provide a
completed set of provisions with their offer found at FAR 52.212-3,
Offeror Representation and Certifications-Commercial Items. Provisions
and clauses found at 52.212-1, 52.212-4, 52.212-5 apply. The following
FAR and VAAR clauses and provisions are added by addendum 52.224-1
Privacy Act Notification; 52.224-2 Privacy Act; 852.237-7
Indemnification and Medical Liability Insurance; 852.270-4 Commercial
Advertising; and 852.271-70 Services Provided Beneficiaries. The
duration of required services is a minimum of ninety (90) calendar days
up to six (6) months. Offerors may find the FAR provisions and clauses
on the Internet at www.arnet.gov/far/. Proposals will be accepted
immediately, but no later than August 18, 2000. Due to the critical
need of these services, award may be made prior to August 18, 2000. An
official authorized to bind the offeror's organization must sign all
