Medical Logistics Office/Veterans Affairs Special Services (AFMLO/VASS)
Contracting Activity has a requirement to provide an Automated
Appointment System for the 305th Medical Group, McGuire AFB, NJ. This
is a combined synopsis/solicitation for commercial items prepared in
accordance with FAR Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation, proposals are being requested and a written
solicitation WILL NOT be issued. Solicitation number
RFQ-797-FDF4-00-0090 is issued as a Request for Quotation. The
solicitation document and incorporated provisions and clauses are those
in effect through FAC 97-19. The SIC code is 3661 and the small
business size standard is 1000 employees. This action is set aside for
small business. This RFQ is for brand name only. The contractor shall
provide the following: Quantity of 1 each Part No. 9899-30-A
ONTAS/Resource, NT platform, which supports ONTAS/IVRE
ONTAS/Appointing; Part No. 9899-32-A, ONTAS/Interactive Voice Response
Engine; Part No. 9899-70-12-A ONTAS/Telephone, telephone line card,
which supports voice-prompts being played to callers; Part No.
9899-31-A ONTAS/Appointing, software module to customize interaction
with the CHCS appointing module; and Quantity of 4 each Part No.
9899-31-A-1, software module customized to support 4 different types of
GYN appointments and interaction with the CHCS appointing module. Once
a purchase order has been awarded an implementation plan is required
to be developed with an existing software developer to coordinate
interface requirements. Delivery is FOB destination to McGuire, AFB,
NJ. Set up cannot begin prior to 18 September 2000 and the system must
be operational no later than on/or about 29 September 2000. The
following FAR clauses apply to this acquisition. FAR clause 52.212-1,
Instructions to Offerors -- Commercial Items; FAR Clause 52.212-4,
Contract Terms and Conditions -- Commercial Items; FAR Clause 52.212-3,
Offeror Representations and Certification -- Commercial Items; and FAR
Clause 52.212-5, Contract Terms and Conditions Required to Implement
Statutes or Executive Orders -- Commercial Items. Offerors shall
include a completed copy of the provision at 52.212-3, Offeror
Representations and Certification -- Commercial Items, with the offer.
