is a combined synopsis/solicitation for commercial service prepared in
accordance with the format in FAR Subpart 12.6 and 13.5 as
supplemented, with the additional information included in this notice.
This announcement constitutes the only solicitation; This is a Request
for Quote and a written solicitation will not be issued. Solicitation
number RFQ 667-01-1-100-0012; award will be made using the simplified
acquisition procedures. This solicitation document and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular 97-18. This acquisition is unrestricted. The standard
industrial classification (SIC) 7699, North American Industrial
Classification System (NAICS) 811219. REQUIREMENT: Contractor agrees to
provide technical assistance for the following Philips X-Ray equipment
during the period of 10/1/99 through 9/31/01: D97 SCP Gen./with Bolus
Chase EZ-Vision/DSI (EZ-DESK), HORZH/SCP80, D76 + SCP80 Gen., Cath
Poly C / OM220 Gen. Technical Support: During period of contract the
customer will have telephone access to help desk for Bio Med Engineers
during normal business hours. On site assistance to be provided. Labor
and replacement parts to be 20% discount. Replacement parts to be
Philips manufactured. Discount of 25% on tubes, intensifiers, camera
pickup tubes, tetrodes, magnetrons, thyratrons, cryogens, magnets and
transducers. 5. TERM OF CONTRACT: This contract is effective for twelve
months from October 1, 2000 to September 30, 2001. 6. WORK HOURS:
Monday through Friday, 7:00 a.m. to 4:30 p.m. b. National Holidays: The
ten (10) holidays observed by the Federal Government, i.e., New Years
Day, President's Day, Martin Luther King Jr.'s Birthday, Memorial Day,
Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving
Day, and Christmas Day. 7. QUALIFICATIONS: Personnel under this
contract shall be licensed or certified by the manufactures to perform
services in the Phillips Equipment. The qualifications of such
personnel shall also be subject to review by the VAMC Chief Biomedical
Engineer and approved. 8. PAYMENTS: Sums due for actual work done, the
Contractor will be paid monthly, in arrears, upon receipt of a properly
prepared invoice FAR 52.232-25. The Contractor shall submit all
invoices to the following address: Chief Fiscal Service (04) Overton
Brooks VA Medical Center 510 East Stoner Ave. Shreveport, LA 71101. 9.
CONTRACT PERFORMANCE: a. The contractor will be monitored through a
variety of mechanisms, including but not limited to: -- sign-in/out
log. -- Chart documenting repairs. -- QA monitors designed to assess
the quality of service provided. b. The Chief of Biomedical Engineering
will be responsible for monitoring the quality of services provided
under this contract. 10. CLAUSES The provisions at FAR 52.212-1,
Instructions of Offeror -- Commercial Items (Mar 2000) applies to this
acquisition. There is not an addenda to these provisions. The clauses
at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items
(May 1999) applies to this acquisition. There is an addenda to this

by Reference (Feb 1998), This contract incorporates one or more clauses
by reference, with the same force and effect as if they were given in
