202-231-8460 The Defense Intelligence Agency intends to procure from a
single source contractor, The Zoldak Group, 7209 Burtonwood Drive,
Alexandria, Virginia, 22307-2021. Description of services: The primary
purpose of the work is to streamline the accident investigation and
documentation process to better serve the DIA. Automating the accident
investigation process will save time and effort to in investigating
and abating accident causes. It will enhance the ability to perform
trend analysis and developed the best matrixes for safety performance
measurement. Finally, the automation process will be linked to the
Facility Management Assistant (FMA) system that documents and monitors
abatement of hazards for DIA. The vendor will develop the accident
investigation system in three phases. Phase 1. Identify and Develop
Requirements (six weeks). The vendor will meet with the DIA Safety
Officer to establish the goals and objectives for the proposed software
and outline the overall strategy for development. Reports,
methodologies, and interlinks will be communicated during this process
to ensure that the vendor and safety officer agree on the conceptual
diagram for this product. The contractor will prepare the requirements
paper that will outline software development and capabilities. The
paper will include details in the database management, interactive
capabilities with FMA and Microsoft Office, reports (accident report,
trend analysis reports, and performance matrixes). The paper may
further explore additional advantages to DIA through discussions
between the vendor and Safety Officer. Phase 2. Phototype development
(six weeks). The vendor shall develop a software accident investigation
prototype for testing. The prototype will be security fenced with
write, edit, and read privileges (other privileges may be established
and phase 1). Testing will include loading accident data into the
accident report windows, running numerous reports on trends analysis
and matrix measurements, and compability links with FMA and Microsoft
Office. Phase 3. Delivery of the Accident Investigation System (three
weeks). The final software package will be a server based product, with
a license for a maximum of 20 simultaneous users. Licensing agreement
will be pursued using DIA's current license agreements with Microsoft,
Oracle, Crystal Reports, and any other applicable programming product.
The product will have security applications to protect information that
falls under the Privacy Act. The Federal Supply Code (FSC)is
R706-Logistics Support Services. The Standard Industrial Classification
