commercial items prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; quotes are
being requested and a written solicitation will not be issued.
Solicitation/RFQ number 57-08-00 applies. It is anticipated the
requirement will not exceed the Simplified Acquisition Threshold. This
solicitation document incorporates provisions and clauses in effect
through Federal Acquisition Circular 97-16. This procurement is a 100
percent set-aside for Small Business concerns per FAR 52.219-1 and
52.219-6; all eligible, responsible sources may submit an offer. The
standard industrial classification code is 5032 and the business size
standard is 500 employees. This requirement is for a fixed priced
contract to furnish, deliver and tailgate spread stone, VDOT grade 25,
Crusher Run to three sites located on the George Washington &
Jefferson National Forest, James River Ranger District, Covington,
Virginia 14426. Line Item 1 is approximately 3,600 tons to Sweet Road,
FDR 350. This road is 10.4 miles long and will have stone (VDOT grade
25, Crusher Run) placed to a depth of 2 inches for the entire length.
The Sweet Road can be located by traveling south from Covington,
Virginia on State Route 18 to Boiling Springs, Virginia. In Boiling
Springs turn right on to State Route 613. Follow State Route 613 to the
intersection of FDR 350 Sweet Road on left. The Sweet Road continues to
intersection with State Route 18. Line Item 2 is approximately 50 tons
to McGraw Hollow Road, FDR 336. This road is .2 miles long and will
have stone (VDOT grade 25, Crusher Run) placed to depth of 2 inches for
the first .2 miles. The McGraw Hollow Road can be located by traveling
north on State Route 629 from Interstate 64 at exit 27. After passing
the entrance sign to DOUTHAT State Park, the McGraw Hollow Road will
be the first road on the left. Line Item 3 is approximately 50 tons to
White Oak Flats Road, FDR 338. This road is 4 miles long and will have
stone placed at spot locations along the length of the road. Stone
(VDOT grade 25, Crusher Run) placed to a depth of 2 inches in the areas
marked for stone placement. To locate the White Oak Flats Road follow
Rose Street from Clifton Forge, Virginia until it turns into State
Route 606. Follow State Route 606 approximately 2 miles to the
intersection with FDR 338, which will be on the left at the top of the
first hill. Stone will be delivered 30 days after receipt of order.
The successful offeror will be required to place a minimum of 200 tons
of stone per day until the entire tonnage has been placed on the
roads. Stone will be tailgate spread to a depth of 2 inches. A Forest
Service inspector will be on site during tailgate operations to ensure
the stone is placed on the correct road and/or section of road. Forest
Service inspector will also collect weight tickets as stone is placed.
Provision 52.212-1, Instructions To Offerors -- Commercial Items,
applies to this acquisition. Under this provision the statement in the
second sentence of paragraph (a) is to read "Item 17 to the CBD
combined synopsis/solicitation" rather than "Block 10 of the
solicitation cover sheet (FS 1449)". The second sentence of paragraph
(b) and the first sentence of paragraph (b)(11) should read SF 18
rather than SF 1449. The government intends to make a single award to
the responsible offeror whose offer is the most advantageous to the
government considering price and price related factors. Provision
52.212-2, Evaluation -- Commercial Items, applies with paragraph (a)
completed as follows: Award will be made to the offeror that meets the
solicitation's criteria for technical acceptability, price and past
performance. Clause 52.212-4, Contract Terms and Conditions --
Commercial Items applies to this acquisition; there are no addenda.
Offerors are required to complete and include a copy of the following
with their offers: FAR 52.212-3, Offeror Representation and
Certifications -- Commercial Items; FAR 52.219-1, Small Business
Program Representations; FAR 52.222-22, Previous Contract and
Compliance Reports and FAR 52.225-1, Buy American Certificate. The
clause at 52.212-5, Contract Terms and Conditions Required to
Implements Statutes or Executive Orders -- Commercial Items applies
with the following applicable clauses for paragraph (b): FAR 52.222-26,
Equal Opportunity (E.O. 11246); FAR 52.222-35, Affirmative Action for
Special Disabled and Vietnam Era Veterans (38 U.S.C. 4212); FAR
52.222-36, Affirmative Action for Handicapped Workers (29 U.S.C. 10).

Prohibition of Segregated Facilities and 52.232-33, Payment by
Electronic Funds Transfer -- Central Contractor Registration. The full
