synopsis/solicitation for commercial items prepared in accordance with
the format in Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; Requests for Quotations (RFQ) are being requested
and a written solicitation will not be issued. This solicitation is
issued as an RFQ using Simplified Acquisition Procedures (SAP)
authorized in Subpart 13.5 -- Test Program for Certain commercial items
and is expected to result in the award of a single firm-fixed price
contract. The solicitation document and incorporated provisions and
clauses are those in effect through Federal Acquisition Circular 97-19.
The solicitation is issued as full and open, and all responsible
quoters may submit a quote which will be considered. The Presidio of
Monterey Army Health Clinic (POMAHC, in Monterey, California has an
immediate need to contract for the personal services of one full time
Physician Assistant(PA) and one full timeNurse Practitioner(NP), for
the period of 1 Oct 2000 (or date of award if subsequent thereto)
through 31 Sep 2001 for the Base Year, with four (4) consecutive option

Defense Acquisitions of Commercial items. The following clause within
clause 252.212-7001 applies to this solicitation 252-243.7002 Requests
for Equitable Adjustment. The following provisions apply, SHORT TERM
OPTION a. The contract may be extended, at the Government's option, for
a period of up to three (3) months, exercisable in increments of not
less than one (1) month. If the contract contains an unexercised
option, this short term extension shall be subtracted from the total
duration of the immediately succeeding option period that may follow as
a result of the exercise of "Option to Extend the Term of the Contract"
contained in this Addendum so that the combination of the short term
option(s) and the annual option will not exceed 12 months duration. b.
If there is an immediately succeeding option period, the prices paid
during the short term option(s) will be the unit prices for the period
immediately succeeding option period. If there is not an immediately
succeeding option period, the prices for the short term option(s) will
be the unit prices of the preceding contract year. c. This option may
be exercised by the Government by unilateral modification at least 7
calendar days prior to the expiration of the contract period, as such
period may have been extended; provided that the Contracting Officer
shall have given notice of the Government's intention to exercise at
least 15 calendar days before this contract is to expire. If the
Government exercises this option, the contract as renewed shall be
deemed to include this option provision, and 52.217-9 Option to Extend
the Term of The Contract, insert into paragraph (a) within 15 calendar
days prior to expiration; provided that the Government gives the
Contractor a preliminary written notice of its intent to extend at
least 30 calendar days before the contract expires; 52.212-1

Evaluation Commercial items, as follows: (1) Offers will be evaluated
based on submission of Evidence of Availability of Qualified Staff.
Offerors shall provide credentials for each person proposed to perform
services at Madigan Army Medical Center (MAMC) for evaluation and
approval. (2) Past Performance- Provide references with whom recent
past performance can be verified. Include the contract number, dollar
value, name, title, phone number of a person with whom to speak
regarding your performance, and a summary of the scope of work. You may
submit any other information regarding specific contracts that you deem
relevant to the evaluation of your firm's past performance. Information
regarding past performance that is obtained from other sources may be
considered by the Government as part of the evaluation process.
(3)Price. The Government intends to evaluate offers and award without
discussions. However, the Government reserves the right to conduct
discussions if determined to be in the its best interest. The closing
date and time for receipt of quotations is 25 Aug 2000 at 10:00 AM PT.
Method of submission of proposals is at the offeror's discretion, they
may be submitted on letterhead stationary via regular or express mail,
by facsimile to (253) 968-4922 or via e-mail to
ruth.richmond@nw.amedd.army.mil. Proposals shall include the RFQ
number, the hourly rate per specialty, the total price for both
specialties for each year and the total price for all years. Full text
