Award will be made using a combination of FAR Parts 12, 13, & 15. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number RFQ 249-00-10 is being issued as a request for quotation (RFQ) which incorporates provisions and clauses though Federal Acquisition Circular 97-19. This solicitation is a total hubzone small business set-aside. The SIC code is 7363. Base year performance October 17, 2000 to October 16, 2001, with renewal option for one (1) additional year. Contractor shall quote an all inclusive hourly rate covering direct and indirect costs including holiday, vacation, health and welfare. Staff must be present at Consolidated Mail Outpatient Pharmacy, 5171 Sam Jared Rd, Murfreesboro, TN 37130 (CMOP) and be performing the required services in order to receive payment. All positions/shifts include an unpaid thirty minute lunch/dinner break during work hours. The above number of staff are estimates only and may increase or decrease during the contract period at no penalty to CMOP. The Government anticipates award of a requirements contract. Only one award will be made. The contract employees will be required to observe all CMOP policies and memorandums. The Government reserves the right to remove any contractor personnel not conforming to CMOP policies. General Clerk II duties: a. Clerks are required to support the operation of conveyor-driven tote and puck traffic and to take individual labeled prescriptions from aplastic tote, scan the barcode prescription label and place the prescription and computer printed patient refill request sheets in a mail bag or box. Clerk is required to apply preprinted mail label and barcode is scanned. Prescription labels are checked for appropriate patient name to match mailing label with appropriate document. Procedure is repeated. Individual to be supervised at all times. b. Clerks are required to keep their area clean and maintain stock for their area, replace toner, clean printer as needed, and replenish other supply items when needed. c. Clerks are required to keep totes circulating on or off main line as an ongoing process. d. Clerks are required to keep sorting lanes cleared and checked as appropriate, run mail conveyor, and load stock to truck as needed. e. Clerks may be required to assist with labeling, accurately and per the production standard of the operating system, of bulk items, prepacks or other stock that has been received prior to being packaged/labeled. f. Clerks may be required to assist in the repackaging of bulk stock into unit of use package sizes when needed. General Clerk III duties: a. All of the duties listed above for General Clerk II are to be performed by the General Clerk III. b. Clerk III are required to stock and maintain stock levels for the dispens-a-matic dispensing system in an accurate and timely manner. Fork Lift Operator duties: a. All of the duties listed above for General Clerk II and General Clerk III are to be performed by the forklift operator. The forklift duties will only take a small portion of the scheduled day shift (30%) and the remaining hours may be utilized in either the General Clerk II or General Clerk III. b. Operates a manually controlled gasoline, electric or liquid propane gas powered Government Owned forklift to transport goods and materials of all kinds about a warehouse or other establishment. c. After contract award, the contractor shall obtain and maintain forklift operator certification on Government Owned equipment at no cost to CMOP. Forklift operators must be fully insured by contractor. d. A back up certified forklift operator must be available if the regularly scheduled operator is absent. The back up operator will only be paid at the forklift pay rate for the days in which work was performed. Knowledge and Skills Required: (Qualifications) for General Clerk II, General Clerk III and Forklift Operator: a. Ability to read English; Must be 17 years old and a High School Graduate or have GED; Ability to stand long periods, lift, twist, and stoop; Manual dexterity to manipulate totes on and off belt and seal packages of all sizes; Ability to move stock and/or packages from one area to another; Forklift operator will be required to maintain OSHA Safety Standards 1910.178. CONTRACTOR'S RESPONSIBILITIES: a. The contractor shall ensure each person performing work under this contract has the appropriate training and/or education. CMOP is not liable for injury either personal or property to the contractor or his/her employees that mayoccur during the performance of the contract. The contractor agrees to procure and maintain for the duration of the contract period, Personal Liability and Property Damage Insurance, General Liability, Workman's Compensation and Employers Public Liability Insurance in accordance with the laws of the State of Tennessee, and must provide certificate to Contracting Officer prior to commencing work. b. The contractor shall ensure proper dress and conduct of employees. Uniforms are not required. Supervision of contractor employees is the responsibility of the contractor. c. The contractor is responsible for meeting applicable standards in the current edition of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) Comprehensive Accreditation Manual for Long term Care Pharmacies. These standards include, but are not limited to: Follow all CMOP policies related to infection control and safety; Maintain a job description for each classification of personnel provided under contract.; Verify applicable education and training, licensure, knowledge and experience appropriate for assigned responsibilities and review of references prior to initial hire.; Conduct annual performance evaluation of each individual provided under contract.; Provide annual training in infection control, safety, and roles and responsibilities during a fire or other emergency. Ensure all personnel are trained when training sessions are held.;Implement quality improvement monitoring to improve the services provided.; Abide by the CMOP standards of quality for accuracy, timeliness, volume, and cost.; Contractor shall provide on-site administration personnel to monitor daily attendance (including timekeeping), provide all training and orientation of new staff, monitor conduct, handle payroll issues, schedule appropriate number of individuals as required, coordinate overtime, and any other performance issues noted by the COTR. Contractor or on site administration personnel will handle all recruitment matters including applications. Onsite administration must be available during both shifts and provided with appropriate equipment for their use in daily administration duties, such as, computer or laptop, timekeeping equipment, fax, cellular phone, etc. On-site administrator is to be provided at no direct cost to CMOP and is not included in the positions identified above. WORK HOURS: a. First Shift: 6:30 am to 3:00 pm; Second Shift: 3:30 pm to 12:00 am, Monday to Friday. Overtime will be required on occasion with prior notification to the on site contract supervisor. Overtime hours are normally worked on weekends but may be utilized by extending shift hours by one hour. b. There are ten Federal holidays and it will be necessary to work on some Federal Holidays. CLAUSES The provisions at 52.212-1, Instructions of Offeror -- Commercial Items (Mar 2000) applies to this acquisition. ADDENDUM TO 52.212-1: Contractor must submit 3 copies of their quote. EVALUATION FACTORS: 52-212-2 Evaluation-Commercial Items: Contractor experience will be evaluated as it relates to the services requested. Award will be made to the company whose quote demonstrates the best value to the Government. Quotations will be evaluated on the following criteria, ranked in order of importance. Factor I Past Performance :Quotes must identify existing or past contracts (completed within past year) as evidence of their experience and for each contract provide at least two points of contact with telephone numbers or provide reference letters. Reference letters must be verifiable and should include similar contracts held by the contractor with government and industry. The quote must contain a minimum 3 references. Factor II Technical A detailed plan shall be submitted describing the contractors ability to furnish required personnel. The narrative shall address intent to meet Contractor Responsibilities noted above and at a minimum; recruitment, onsite management, Drug Free Workplace Policy, Back Up Personnel, Absenteeism, Timekeeping, ability to meet training, CMOP, OSHA and JCAHO Standards. Factor III Price. The lowest evaluated price will receive the full points. The remaining quotes will receive points in proportion to the price quoted in comparison to the lowest price. The past performance and technical factors are listed in descending order of importance and when combined are equal to price in importance. NOTE: Quoters are advised they are not restricted to what is presented in their quotes as long as sufficient material is provided to allow evaluation of specific elements of quotes as defined above. To ensure proper consideration and evaluation of your quote, you are advised to fully outline your intent to meet or exceed our requirements. The Government reserves the right to award without discussions. Therefore, each initial quotation should contain the quoter's best terms and conditions from a price and technical standpoint. Dept of Labor Wage Determination 1994-2497, Rev 15, dated 6-4-00, classification Gen Clerk II hourly rate is $7.87, Gen Clerk III hourly rate is $8.86, Fork Lift Operator hourly rate is $$9.84, plus health and welfare of $1.92 an hour, is applicable. Wage Determination dictates: 2 weeks paid vacation after 1 year of service with a contractor or successor; 3 weeks after 10 years; 4 weeks after 15 years. Length of service includes the whole span of continuous service with the present contractor or successor, wherever employed, and with predecessor contractors in the performance of similar work at the same Federal facility. Holidays: Minimum of ten paid holidays per year: New Year's Day, Martin Luther King Day, Washington's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans' Day, Thanksgiving Day, and Christmas Day. (A contractor may substitute for any of the named holidays another day off with pay in accordance with a plan communicated to the employees involved.)
