This request for quotation (RFQ) is combined/synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in this notice. The solicitation number is DABT57-00-Q-1012 and is issued as a request for quotation (RFQ). This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The products and services requested are: Fit-To-Win Program Coordinator for the Period 01 October 2000 thru 30 Sept 2001 with four (4) option years if exercised to run from 01 October through 30 September of each year. Hrs to be worked between the hours 0730-2100. Location: Anderson Field House and Facilities located on Fort Eustis and Fort Story associated with Activities and Testing. Government will supply all equipment and supplies to include duplicating.Contractor will bill the Government for total #of service hrs provided.QUALIFICATIONS:Education: Bachelors degree in Education, Nutrition, Biology, Health, Physical Education or related field, i.e. Life Sciences, Recreation, etc. Bachelors degree in any field plus two years specialized experience and one year general experience. General Experience: Minimum of six (6) months experience in military fitness programs (i.e. active duty military service, civilian work experience dealing with physical fitness or recreation programs) Specialized Experience: A minimum of two years experience working as a Certified Personal Trainer or Fitness Specialist with credentials from one or more of the following organizations: ACSM (American College of Sports Medicine), CIAR (COOPER INSTITUTE FOR AEROBIC RESEARCH, ACE (American Council on Exercise), NSCA (National Strength and Conditioning Association) IDEA (International Association of Fitness Professionals).SPECIFICATIONS: Provide Two (2) page 8 x 11 inches Health Promotion Articles by the 15th of each month in draft layout for publication in the DPCA Community News. Articles will include nutrition/fitness/wellness information/tips, smoking information, heart disease information, weight control Information and other Health Promotion information. Coordination of these articles Must be made with hospital i.e. Nutrition/Care, Physical Therapy, Preventative Medicine. Maintain data base on Fit To Win participants for Program evaluation purposes. Advertise, instruct and maintain Fit To Win Walking Program, Run for Your Life Program and Swim for Your Life Program. Coordinate Wellness Incentive Program, Civilian Fitness Program and Fire Fighter Program. Compile and draft information handouts on nutrition, fitness, running/ walking/biking opportunities on Fort Eustis and surrounding Community. Administer various fitness tests (i.e. skinfold,YMCA step test, BP screen, u/w weighing, etc.) Participate in the administration of Computerized testing, exercise prescription and equipment orientations.Cost Per Hour $ based on requirements of Physical Fitness Program NTE 40 Hours (unless requested in advance).
