straight-sided HDPE white bottles, 200cc HDPE white bottles and child
resistant, 38 mm closures to fit both bottle sizes. The items are for
usage by Department of Veterans Affairs Consolidated Mail-Out
Pharmacies (CMOPs). In addition to filling prescriptions for our
nations veterans, these facilities also prepackage prescription
medicines for other entities for a fee, as authorized by the Veterans
Health Care Eligibility Reform Act of 1996, Public Law 104-262. This
contract is intended to provide the primary source for the bottles and
closures that are processed on Modular Packaging Systems prepackaging
machines at the CMOP facilities located in Lancaster, TX and N.
Charleston, SC. Orders will be placed on as as-needed basis under this
indefinite delivery-indefinite quantity requirements contract for a
period of one year after date of award. Only one contract will be
awarded as a result of this solicitation. The contract includes
pre-priced options for two additional years, not to exceed three years.
Estimated requirements for the initial year include approximately 9
million-120cc bottles, approximately 75,000-200cc bottles, and
approximately 9 million closures. It is also anticipated that some of
the quantities will increase the second and third years. Offerors are
also invited to propose tiered pricing, full or half tractor-trailer
loads or other discount pricing. The successful contractor will provide
storage of 120cc bottles for the CMOP facility located in Lancaster,
TX. The bottles and closures must meet exact dimensions shown on
solicitation drawings. Bid samples are required with USP test results
from each batch provided. The bottles and closures must allow smooth
operation on the prepackaging machines, without hanging up, binding,
jamming, or causing any damage to the machine or products. For the TX
CMOP, same day delivery will be required for 120cc bottles and their
closures, and 24 hour delivery for the 200cc bottles and their
closures. The SC CMOP will place orders monthly that will specify
weekly deliveries needed. Contract allows 30 calendar days after award
of contract for contractor to build adequate inventory. All items
shall meet FDA requirements. The contract will be firm-fixed price with
economic price adjustment. The Government will award a contract
resulting from this solicitation to the responsible offeror whose offer
conforming to the solicitation will be most advantageous to the
Government, price and other factors considered. The following factors
shall be used to evaluate offers: (1) All minimum requirements must be
met for consideration of other factors. Minimum requirements include:
Acceptability of samples and USP test batch results, FDA approval of
raw materials, and compliance with FDA requirements as they pertain to
preserving the integrity of the 120cc bottles while in storage. (2)
After determination that an offer has met the minimum requirements, the
offer will be evaluated for the following factors: a. Pricing, which is
the most important; b. Past Performance, c. Storage plan, and d.
Quality assurance plan, all of which are equal in importance. Price
evaluations will include HUBZone and Small Disadvantaged Business
factors, if applicable, evaluation of options, and where the Government
determines it can comply with their provisions, any special discounts
offered. All evaluation factors other than cost or price, when combined
are significantly more important that cost or price. This acquisition
is unrestricted as to business size. Standard Industrial Code (SIC)
3085, Manufacturers of Plastic Bottles, applies to manufacturing firms
and the small business size standard is not to exceed 500 employees.
SIC 5162, Wholesalers of Plastics Materials & Basic Forms and Shapes,
applies to wholesale or distributor firms and the small business size
standard is not to exceed 100 employees. Solicitation documents will be
