R -- PERSONAL EMERGENCY RESPONSE SYSTEM FOR HOME-BOUND VETERANS SOL RFQ600-103-00tmt DUE 090700 POC Teresa, Thomas, Contracting Officer, FAX (562) 961-1377 It is the intent of the Government to award a sole source firm fixed-price contract to Lifeline Systems of Cambridge, Massachusetts, to provide for a personal emergency response system that will give medically frail and elderly veterans emotional security in their homes. The service consists of a monitoring system and an alert device. These devices are one of two types. The first attaches to the phone, the second is an emergency alert button worn by the veteran. When a signal is sent it alerts an operator of the veteran's need for assistance. The operator contacts a designated individual or facility on a pre-arranged list. The Contractor shall immediately contact, via FAX, the social Work Service for all emergency situations to inform the Government on the current status of the veteran. A detailed monthly report of all contacts shall also be prepared and forwarded to the Social Work Services. The contractor shall install the devices and provide instructions. This service includes the initial set-up of equipment, as well as the input of the veteran's emergency data. A two- (2) year warranty on the equipment shall be included to cover all hardware. The period of performance shall be for one (1) year from the date of award with the option to renew the contract for four (4) additional one- (1) year periods. It is necessary that the system utilize the Lifeline Systems devices which are currently in place in veterans' residences, as well as meet the specialized needs of the veterans and Social Work Service at the San Diego Healthcare System, 3350 La Jolla Village Drive, San Diego, California.
