combined synopsis/solicitation for commercial items prepared in
accordance with the format in FAR Subpart 12.6 as supplemented with
additional information included in this notice. This procurement is
conducted under simplified acquisition procedures in accordance with
FAR Part 13. This announcement constitutes a formal Request for
Proposal (RFP) and a written solicitation WILL NOT be issued. The
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition 97-19. RFP 658-60-00 should be
referenced on all proposals. The Standard Classification (SIC) for this
procurement is 8042 and the small business size standard is gross
annual receipts NTE $5.0 million. This procurement is unrestricted. The
VA Medical Center, 1970 Roanoke Boulevard, Salem Virginia 24153, has an
emergent need for Off-Site Optometry/Optical Services for veteran
patients and eligible beneficiaries. VAMC Salem, Virginia intends to
solicit proposals and award a firm, fixed price contract under the
authority of Title 38 USC Section 8153. The contract is for a ninety
(90) calendar day period but may be extended for an additional ninety
(90) calendar day period. Contractor shall be proficient in the English
language, both spoken and written. Optometry/Optical Services to be
furnished off-site in the local community at the service providers
facility shall consist of the following: (I) Optometry Services; (a)
Provide detailed diagnostic, therapeutic and rehabilitative procedures
necessary for the management of 1) Refractive anomalies, 2) Problems
of binocular vision, ocular motility, accommodation of visual
perception, 3) Other complex visual and ocular anomalies including
aphakia, keratoconus, aniseikonis and other related anomalies. (b)
Perform procedures necessary for the diagnosis and management of ocular
disease and ocular manifestations of systemic disease, including the
use of pharmaceutical agents. (c) Complete part II, sections 4A 4G, 5A
as indicated, 12 and 13 of the prescription and authorization for
eyeglasses, VA Form 10-2914(R). (d) Patients in need of advanced
medical or surgical eye care services (outside the scope of general
Optometric practice), laboratory or radiology testing shall be
discussed with Dr. Dan Eckermann, Chief, Optometry Section, VAMC Eye
Clinic, phone number (540) 982-2463, extension 2831. (II) Optical
Services; (a) Measure interpupilary, distance and at near, to determine
centration of the lenses and bifocal height. (b) Utilizing eyeglass
frames and ordering guidelines provided by VAMC, fit frame to face of
patient and assist patient in selecting proper frame for face and
visual needs. Complete Part II, Section 5B -- 11C of VA Form
10-2914(R). In order to expedite ordering of eyeglasses; fax a copy of
VA Form 10-2914(R) to VAMC Prosthetics Department at (540) 224-1943.
(III) Payments shall be made in arrears upon receipt of invoice and
original VA Form 10-2914(R) with name of patient, Social Security
Number, date of service and detailed description of services provided.
Invoice and completed VA Form 10-2914(R) shall be submitted to Mickey
Bryarly, Surgical Care Service Line (112), Veterans Affairs Medical
Center, 1970 Roanoke Boulevard, Salem, Virginia 24153. The Contractor
shall be responsible for protecting the personnel furnishing services
under this contract. To carry out this responsibility, the Contractor
shall provide the following; Worker's Compensation Insurance,
Professional Liability Insurance Income Tax Withholding, Social
Security Payments, and Health Examinations. The parties agree that such
personnel shall not be considered VAMC employee(s) for any purpose and
shall be considered employee(s) of the Contractor. The Contractor
shall perform required services in accordance with established
principles and ethics of the medical profession established by the
JCAHO, and the Veterans Health Administration (VHA) regulations, and
the policies, procedures, and regulations of the Medical Staff bylaws
of the VAMC, Salem, Virginia. Offerors shall provide a completed set of
provisions with their offer found at FAR 52.212-3 -- Offeror
Representation and Certifications Commercial Items. Provisions and
clauses found at 52.212-1, 52.212-4 and 52.212-5 apply. The following
FAR and VAAR clauses and provisions are added by addendum 52.224-1
Privacy Act Notification; 52.224-2 Privacy Act; 852.237-7
Indemnification and Medical Liability Insurance; 852.270-4 Commercial
Advertising; and 852.271-70 Services Provided Beneficiaries. Offerors
may find the FAR provisions and clauses on the Internet at
www.arnet.gov/far/. Proposals shall be accepted immediately, but no
later than September 6, 2000. Due to the critical need of these
services, award may be made prior to September 6, 2000. An official
authorized to bind the offeror's organization shall be required to sign
all offers. All proposals should be submitted to Contracting Officer
