Donna Peden 228-563-2689 This is a set-aside for HUBZone small business
concerns. Contractor shall provide medical disposal services to the
Department of Veterans Affairs, VA Gulf Coast Veterans Healthcare
Systems, Biloxi/Gulfport Divisions, MS and Mobile Outpatient Clinic,
Mobile, AL for the Base Period October 1, 2000 through September 3,
2001 with option year I October 1, 2001 through September 30, 2002.
This is a combined synopsis/ solicitation for a commercial service,
prepared in accordance with the format in subpart 12.6, as supplemented
with additional information included in this notice. This notice
constitutes the only solicitation; quotations are being requested and
a written solicitation will not be issued. Solicitation number/Request
for Quotes (FRQ) 520-01-01 is assigned and shall be referenced on any
quotation. This is a Request for Quotation for which it and all
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular 97-77. This procurement is set-aside for HUBZone
small business concerns. The Standard Industrial Classification Code
(SIC) is 4953. SCOPE OF WORK: 1. Contractor is to provide inservice
with VA Staff necessary to establish safe, efficient and cost-effective
identification, separation, segregation, packaging, labeling, and
handling of medical waste. 2. Contractor will supply special containers
and red bag liners for the containment of the waste. Containers to be
made of rigid plastic construction designed to prevent puncture by
needles and other sharps, capacity not to exceed 40-gallons, and
include locking safety handles along with appropriate bio-hazard
warning symbols approved by the State and Federal regulatory agencies.
3. Contractor to provide scheduled pick-up and delivery dates, which
shall be as often as necessary to assure access to the storage areas,
but not less than two (2) times per week. 4. Contractor will make
complete distribution of the waste to a reliable "State of the Art"
facility approved by all regulatory bodies for disposal. 5. Contractor
will assume all liability for the medical waste removed from the VA
properties from the time of pick-up until the total distribution of the
waste by incineration, sterilization, or other disposal method, which
must comply with EPA, OSHA, CDC, ADEM, TNRCC, ODEQ, LADEQ, and/or any
other controlling agency with oversight authority. 6. The Contractor
will provide tracking of the medical waste from the point of pick-up,
through use of an EPA, MDEQ, and MDOT approved manifest system. Vendor
to also provide hard copy documentation (manifest sheets) to the
medical center, indicating time and date of pick-up, time and date of
incineration and weight of each container received. 7. Contractor shall
provide transportation in compliance with MDOT regulations. Trucks
shall have leak-proof compartments for containing medical waste and
shall have built-in spill control technology. Contractor shall provide
contingency plans for emergency/hazmat response and provide a full
