Description
The Centers for Disease Control and Prevention (CDC) has a requirement for a contractor to provide timely, high-quality telephone sample records to be used by health departments in the U.S., District of Columbus, Guam, Puerto Rico and the Virgin Islands. The database of working telephone numbers will be used to conduct the Behavior Risk Factor Surveillance System (BRFSS) Survey in addition to health related surveys in conjunction with the states, surrounding territories and the CDC. The database shall contain sampling frame information needed to properly weight sample data and shall allow BRFSS states with an enhanced capability to conduct monthly digit-dialed telephone surveys. The sample of database numbers shall be suitable for conducting project-based (which may include geographic stratification within projects), random digit-dialed (RDD) telephone surveys using list-assissted disporortionate stratisfied sampling (DSS) design, sample telephone records for RDD telephone surveys, which may include internal geographic stratification, sampling from information needed for weighing geographic and density stratum for the projects for which sample records are provided, other information related to the production of the sample or changes which may impact the use of the sample by projects or BSB in processing the survey data and technical expertise relative to the production of the sample and consultative capacity on sampling issues. The period of performance for the proposed contract shall be a one (1) year contract with four (4) one year option periods. The government anticipates making a single award in response to this Request for Proposal (RFP). The acquisition shall be acquired using FAR SubPart 15.101-2 _ _Lowest Price Technically Acceptable Source Selection Process_ procedures. Offerors are advised the this RFP is set aside for small business. SIC for this acquisition is 4813. The date specified for receipt of proposals will be approximately 30 days after the RFP issue date which is projected on or about September 15, 2000. Offerors may obtain a copy of the RFP via the internet at www.eps.gov; however, if a hard copy is desired, only written written request citing RFP number 2001-N-00001 shall be honored.
