****The following solicitation is hereby modified to state the following _ The period of performance for this purchase order has changed to one basic year starting 1 October 2000 through 30 September 2001, plus 4 one-year options. Also being amended is the statement of work. The 55th Wing Ehrling Bergquist Hospital reviewing credentials of all potential applicants will determine evaluation criteria. Award will be based on recommendation. The new response date for this solicitation is 12 September 2000/4pm CST. Also the following clauses are hereby incorporated _ FAR 52.217-4, Evaluation of Options Exercised at Time of Contract Award. FAR 52.217-5, Evaluation of Options. FAR 52.217-8, Option to Extend Services. FAR 52.217-9, Option to Extend the Term of the Contract.****This is a combined synopsis/solicitation for services non-personal issued by the 55th Contracting Office, Offutt AFB, Nebraska. This document was prepared in accordance with the format found in the Federal Acquisition Regulations Document (FAR) subpart 12.6, as supplemented with additional information included in this notice. This electronic announcement is the only solicitation being issued. A WRITTEN SOLICITATION WILL NOT BE ISSUED. This is a 100% small business set-aside. Solicitation number(s) F25600-00-T-0180 is being issued as a request for quotation (RFQ). The solicitation document and incorporated provisions/clauses are those in effect through Federal Acquisition Circular (FAC) 97-15. The government intends to issue a purchase order for the following service(s): -- MARRIAGE COUNSELOR FOR THE MENTAL HEALTH DEPARTMENT -- See attached Statement of Work. The period of performance for this purchase order will be 1 October 2000 through 30 September 2001. All services to be performed should be included in the quotation because NO PARTIAL QUOTES WILL BE CONSIDERED. All quotes should be received by 28 July 2000/4pm,CST and, if possible, sent back electronically via e-mail, if possible.
