- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional formation included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-19. The NAICS code is 54133. This is a 100% small business set aside. HVAC AND MAINTENANCE SERVICES: Contractor will take responsibility for the maintenance and repair of the premises including testing and inspection of appropriate equipment and systems. This will be done in accordance with each respective piece of equipment's operations manual, applicable codes, and inspection certificates will be displayed as appropriate. Maintenance will be performed at a rate of twelve (12) hours per week, which will include: 1. Three walk-throughs per week. 2. Twenty-four hour per day, seven days per week emergency response to the building. 3. Routine preventive maintenance for equipment as required by the Operating Maintenance Manuals. All necessary materials and supplies that are required will be ordered by Contractor and billed on a cost plus basis. Should APHIS desire they can arrange to procure the materials, we will still inventory on hand materials and provide a list for ordering. We will, if requested work with APHIS on a master monitoring and control plan for the ARB and centralized in the Wildlife Sciences Building to facilitate site wide Monitoring. Contractor will inspect all portable Fire Extinguishers in the ARB annually in accordance with NFPA Standard 10. Contractor will arrange for repairs, 6 and 12 year maintenance as required and at the expense of APHIS/NWRC. Option Period 2: 10/01/02 through 09/30/03: Item 1: Description: HVAC Maintenance Services: Rate/Month: $_______: Item 2: Time and Labor for Special Projects: Rate/Hour $_______. QUALIFICATION REQUIREMENTS FOR HVAC AND MAINTENANCE SERVICE PERSONNEL: The Contractor shall provide a list of names of key personnel for the HVAC and Maintenance Service. The key personnel must meet the following minimum qualification requirements: a. Proof of a professional license, education or state certification OR a minimum of three (3) years work experience in servicing the type of equipment or system at the ARB building. 2. Approval Process for Maintenance Services and Procuring Material and Supplies, Simplex and Approved Maintenance Companies: a. Weekly maintenance shall be performed as required in each equipment's/systems Operation and Maintenance Manual. b. The contractor must obtain written approval from the COR before acquiring maintenance time and labor services for special projects and emergency repairs. In case of emergency service, overtime is pre-approved only in the event that the contractor was unable to contact the COR and the Contractor's employees use the swipe-card to log in and log out the time of actual emergency hours performed. The contractor must obtain written approval from the COR for any additional maintenance Time and Labor for special projects and emergency repairs. REPORTING REQUIREMENTS: 1. Daily reporting requirements: The contractor shall maintain a daily time sheet to include the name of the employee, the hours worked and the work performed using the weekly activity log. The Contractor's employees begin their weekly activity report each Saturday. The Contractor's employees shall use the swipe card when entering and leaving the ARB building to, log in and log out the time of actual work performed. 2. Weekly Reporting Requirement: The Contractor shall submit to the COR a weekly activity report to reflect the daily time sheets. The COR will report any discrepancies found on the weekly activity report and provide the Contractor with ARB's Remote Card Reader System report. DESIGNATION OF CONTRACT ADMINISTRATOR: The Contracting Officer (CO) has designated Margie Thorson as Contract Administrator (CA). The CA is an Agent of the CO with delegated authority to administer and enforce the terms and conditions of the contract. The CA shall represent the CO on all contractual matters including, but not limited to, performance and/or nonperformance issues, proposed changes, questions pertaining to contract coverage, interpretation of contracts and conditions, negotiating and preparing contract modifications, billing or payment issues, disputes/claims, and other contractual matters that may occur during the life of the contract. CONTRACTING OFFICER'S REPRESENTATIVE (COR) AND ALTERNATE COR. Bob king, the COR, will designate Ann Mannos (970/266-6033), alternate COR during his absence.
