Description
DESC: The FISC Norfolk Acquisition Group is issuing a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are hereby requested. A written solicitation will not be issued. This is issued as a Request for Quote (RFQ), Number N00189-00-T-H835 which incorporates provisions and clauses in effect through Federal Acquisition Circular 97-17. This requirement will be 100% Small Business Set Aside. The Standard Industrial Classification Code (SIC) will be 8322, size standard $5 million. This requirement is for one (1) year from 01 October 2000 through 30 September 20001 with four (4) option years. Please provide prices for the following CLINs : of all services requested): CLIN 0001, 1 Lot, HRO Norfolk, FISC Norfolk, NAB Little Creek, LANTDIV NAS Norfolk, NWS Yorktown & SUPSHIP NN VA; CLIN 0002. 1 Lot, Naval Medical Center Portsmouth, VA. CLIN 0003, 1 Lot, Military Sealift Command Virginia Beach, VA. CLIN 0004, 1 Lot, Public Works Center Naval Station Norfolk. VA.STATEMENT OF WORK/1.the scope of this agreement is to provide employee assistance services for problems concerning marital and interpersonal discord, legal/financial difficulties, alcohol and/or drug abuse, emotional problems, family concerns, and job stress to civilian employees who are provided services by Human Resources Office, Norfolk. This agreement provides for assessment/counseling/referral/diagnostic interview/follow-up services, and supervisory and statistical data under the auspices of the Civilian Employee Assistance Program (CEAP).The Human Resources Office, Norfolk, will be hereinafter referred to as "HRO." The Provider will function as an entirely independent provider of employee assistance. The Provider is not aligned with any one health care organization.2.Scope of Work/Duties of the Provider/A. Provide services as stipulated in this agreement for specified period. Few are to cover population of approximately 12,000 employees nationwide; although most of the employees are within the Tidewater area.B, Serveas contact point for referral and assessment requests from the HRO Civilian Employee Assistance Program Administrator with responsibility for determining the most appropriate referral disposition. This may include securing hospitalization or referral to an appropriate self-help group, outpatient provider-, legal/financial provider- or community service agency.C. Meet quarterly within the HRO CEAP Administrator for the purpose of monitoring program activity, review utilization reports and evaluate ongoing training needs.D. Provide Special Needs Training sessions to be held on an as needed basis with commandsthat are going through downsizing. Emphasis will be placed on dealing with the emotionalimpact of Reduction in Force on employees. Provider will be available for extended periods of time to provide on-site professional counseling sessions, if the need arises.E,. Upon request of the I-MO CEAP Administrator, provide an Employee Assistance Program (EAP) newsletter to be distributed at training sessions and supplied to the HRO CEAP Administrator for distribution as deemed appropriate.F. Provide regularly scheduled work site visits at HRO, one (1) day per week for four (4)hours, by the Clinical Coordinator.G. Require newly hired Clinicians to meet with the MRO CEAP Administrator for HRO Indoctrination.H. Provide free assessment services to all employees (including their dependents and household members) of HRO serviced activities, some which may be out of the local area.I. Refer employees to other resources when services are clearly not within the scope of the agreement. J.Upon request of the HRO CEAP Administrator, provide supervisory training ,&, sessions at a time and location specified by HRO, These are to be conducted for a one-hour period each and will include instructional materials to be approved by the HRO CEAP Administrator. K.Upon request of the HRO CEAP Administrator, provide employee training for employees in high stress positions such as police officers and firefighters. These are to be conducted at a time and location specified by HRO and will include instructional material to be approved by the HRO CEAP Administrator, Additional training will be provided in stress management education awareness and also in supportive debriefings during and subsequent to critical incidents. L.Provide on-going training in the form of educational workshop/conferences, brown bag seminars and public forums, Conduct wellness oriented seminars based upon predominant personal problems presented, needs assessments and recommendations of the HRO CEAP Administrator.M. Provide on-going supervisory/management consultation services as requested on topics of concern, advice on individual cases, job performance documentation and constructive confrontation .N. Assure all Provider's employees possess professional qualifications necessary to ethically and legally perform required services (see Item 4- Qualifications of the Provider's Personnel).0. Assume all legal and professional responsibilities and liabilities attendant on professional practices in the rendering of mental health services. P. Financial arrangements for any treatment or care beyond those specifically contracted for in this agreement are the sole responsibility of the Provider and the employee. HRO Norfolk is not responsible for any costs associated with treatment of employees. Q. Provider will make clear to employees all viable options available to them wh6n further care or treatment is necessary. This includes, but is not limited to, directing them to credentialed /licenced medical, alcohol/drug, psychiatric and legal professionals who accept discounted reimbursement, ascertaining the health benefit plan of the employee to determine what services may be covered, referring them to low-cost community resources when appropriate, and advising them of estimated costs of treatment. R, Selection of referral sources will be based on the Provider's clinical philosophy, scope of services available, ease of access and hours of operation, quality and quantity of professionals available and the Provider's experience with them. S. Provide statistical data for inclusion in semi-annual and annual HRO CEAP reports. This information will not require the use of any identifying details, but is used to measure the relative positive effects of an Employee Assistance Program. T. Provide case management services in the form of follow-up during treatment and post treatment. 3. Confidentiality/- All information received by the Provider will be documented and maintained in accordance with the confidentiality requirements of Public Law 93-282 and implementing Federal .regulations of 42 Code of Federal Regulations as well as Public Law 93-579 (Privacy Act). A.During initial contacts with the HRO Administrator and the Provider's EAP Counselor, the employee will be informed of this policy. Information will be disclosed only with the individual's written consent (see Appendices I and 2). However, in cases evidencing suicidal /homicidal gesturing or child abuse, the proper authorities will be contacted immediately. B.The Provider will be responsible for maintaining all case files in a complete, confidential and secure manner. All such records are considered to be under the jurisdiction of the agency and are to be made available within seven days to the agency's Occupational Health Physician upon request, subject to the above guidelines. Upon termination of this agreement for whatever reasons, records supporting the statistical data reported by the Provider, status of active management and DFWP referrals shall be surrendered to the Occupational Health Physician within 30 days of the termination date.4.Qualifications of the Provider's Personnel/A.Administration and delivery of services will be provided by local community based professions, The Provider's personnel shall consist of Board Certified Psychiatrists, Certified Alcoholism Counselors, Certified Employee Assistance Counselors and Masters and Doctorate level counselors specializing in the treatment of family, marital, substance abuse and related problem areas. All of the professional staff will have expertise in crisis intervention and extensive knowledge of community and referral resources. Upon request of the HRO CEAP Administrator, the Provider will submit in writing within 10 calendar days a listing, of professional staff members and their credentials as described below.B.All personnel serving under this agreement shall be employees or assignees of the Provider who shall be responsible for validation of their qualifications to perform the services described herein. /BOARD CERTIFIED PSYCIATRIST/Duties:1)Provide evaluations and diagnosis, diagnostic assessment and treatment recommendations.2)Provide referral to medical facilities,3)Prescribe medication,4)Provide crisis intervention services,Education.:1)Doctoral degree in General Psychiatry.2) License from a Board of Examiners in Psychiatry,/Experience: Licensed to practice in local medical facilities, Has provided a full range of services to include evaluation, diagnosis and treatment of family, marital, substance abuse and related problem areas. Expertise in crisis intervention and extensive knowledge of community and referral resources./ALCOHOL/SUBSTANCE ABUSE COUNSELOR/Duties:1) Conduct assessment.2)Provide referral for treatment.3)Maintain follow-up during, treatment and post treatment.4)Provide crisis intervention services,5)Conduct special needs &, training and facilitate special needs counseling./Education: State certification as a Certified Substance Abuse Counselor./ Experience: Minimum. of two (2) years of experience in working as a direct provider of chemical dependency services./CERTIFIED EMPLOYEE ASSISTANCE COUNSELORS/Duties:1)Conducts assessments.2)Provide counseling/referral services for individual concerns- marital discord, family concerns, legal/financial difficulties, emotional problems and job stress,3)Maintain follow-up services.4)Conducts special needs training and facilitates special needs sessions.5)Provides crisis intervention services./Education:1)A Masters Degree in Social Work (MSW); and/or,2)Membership in the Academy of Certified Social Workers-, and/or,3)A current state license at an independent clinical practice level (LCSW)./Experience:/1)Not less than two (2) years of professional experience,2)Clinical experience in providing a full range of services to include evaluation, diagnosis and treatment of family, marital, substance abuse and related problem areas. Expertise in crisis intervention and extensive knowledge of community and referral resources./CLINICAL COORDINATOR/Duties:1) Provide regularly scheduled work site visits at HRO one (1) day per week for four (4)hours.2)Conduct assessment and make referral to appropriate counselor.3)Monitor program activity.4)Meet quarterly with HRO CEAP Administrator.5)Provide statistical data for semi-annual and annual HRO CEAP reports.6)Serve as contact point for Employee Assistance Program (EAP) newsletter.5.Eligibility Requirements for Employee Participation/All employees officially on the roles of HRO services activities and their household members are eligible to participate in the CEAP.6. Method of Access/A.Access will be through self referral or by the HRO CEAP Administrator, HRO management personnel are also instructed to make initial contact with the CEAP Administrator to discuss suspected substance abuse or emotional problems prior to or concurrent with any disciplinary action against the employee, If the employee is seeking, "safe harbor" which involves mandatory referral to CEAP, initial contact will also be with the CEAP Administrator.B, Self referrals constitute referrals made voluntarily by the employee or household member. This type of referral is held in the strictest confidence unless otherwise indicated. 7, Hours of Operation/A.The Provider will assure twenty-four (24) hour, seven (7) days a week accessibility by a telephone call. The Provider will have available both a local and a toll free (800) number for this purpose. Any call must result in immediately reaching a counselor or a call-back within thirty (30) minutes. B, The Provider will establish and publicize normal business hours Monday throughFriday. Appointments will normally be scheduled during these hours except when specifically requested at other times or in emergencies./8. Assessment and Diagnostic Interview:/A. Once a call is placed to the Provider's CEAP Coordinator, a preliminary telephone assessment will be conducted. A face-to-face assessment interview will be scheduled within 2448 hours of the initial phone call, when appropriate. In the case of an emergency, work site assessment will be made by the Provider within 30-45 minutes of the call ' Cases of emergency include the following and will be referred for immediate professional intervention; suicidal/homicidal ideation or gesturing, acute substance abuse withdrawal, medical emergencies, acute legal difficulties, acute emotional dysfunction, physical/sexual abuse, deaf/mute referrals .B. During the initial assessment interview a motivational, problem-solving approach will be utilized in order to resolve the presenting problem(s) in an expeditious manner, overcome resistance in the case of mandatory referrals and rapidly determine the most appropriate disposition.C. Assessment interviews will be scheduled Monday through Friday during normalworkIng hours (unless specifically requested at other times) at the Provider's counseling site, or at the work site./9.Referral Disposition/A.Upon completion of the assessment and diagnostic interview, the findings will be discussed with the employee, as well as the recommended helping source(s)- for example, unlimited counseling sessions are available as is referral to an appropriate self-help group, outpatient provider, legal/financial provider, community service agency and/or hospital.B -- The Provider agrees to consider the employee's/dependent's diagnostic needs, health benefit coverage, financial and transportation capabilities and personal preferences whenmaking referrals .C. Insurance plan specialty care providers will be utilized and/or those selected and preferred by the employee and HRO./10. Follow-Up/The Provider will provide follow-up during treatment and post treatment. This follow-up consist of case management, telephone/personal interviews, and confidential mail outs, all in accordance with Privacy Act considerations, Frequency of follow-up will be weekly during EAP counseling and/or treatment. After counseling/treatment, it will be at 2-month, 5-month, 9 month and 12-month intervals. 11. Program Promotion/Publicity:/ the provider's EAP Coordinator will be responsible for coordinating and on implementing all program promotion and publicity in accordance with HRO's policy and procedures. Poster, brochures and wallet cards will also be made available to HRO. Any program materials or promotional activities will be subject to final approval of the HRO CEAP Administrator. 12. Provider Professional Liability and Insurance/ The Provider shall assume all legal professional responsibilities and liabilities attendant on professional practices in the rendering of health services. 13. Termination or Extension of Agreement./A. This agreement is to be in effect 1 October through 30 Sept 2001. B. Cancellation of this agreement of both parties, unless the reason for cancellation is a clear violation of the agreement and rules contained herein. C. Termination of this agreement shall not affect those beneficiaries involved in scheduled assessment appoints, CEAP counseling sessions, and/or ongoing treatment arrangements. D. At the end of the stated period, additional services or an extension of the agreement will be considered, but implemented only after full agreement by both parties regarding fees and any amendment to the agreement.
