- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Reference Request for Proposal No. USAYPG-COMP-INST. This solicitation/synopsis incorporates provisions and clauses in effect through Federal Acquisition Circular 97-19. The Request for Quotation (RFQ) Number is DAAD01-00-T-0025 and the SIC is 8299 ($5 million in annual receipts), this is a 100% small-business set-aside under simplified acquisition procedures, and the FSC is U099. Contract Line Item Numbers (CLIN) are: 0001, Perform Computer Lab and Homework Instruction IAW SOW (period 1 October 2000 through 30 Sept 2001). Per week fixed offer price is $____. 0002, Option , Perform Computer Lab and Homework Instruction IAW SOW (period 1 October 2001 through 30 Sept 2002). Per week fixed offer price is $____. 0003, Option, Perform Computer Lab and Homework Instruction IAW SOW (period 1 October 2002 through 30 Sept 2003). Per week fixed offer price is $____. a. PAYMENT TERMS: The Contractor may submit invoices for no more than two (2) times a month, and shall be paid no more than two (2) times per month in accordance with the terms set forth herein. See paragraphs (g) and (i) of FAR 52.212-4 "Contract Terms and Conditions -- Commercial Items" (MAY 1997). The Contracting Officer may use convenience checks to pay the Contractor. b. ELIGIBILITY: In order to be eligible for this contract, you shall provide the information required in paragraph (1) and (2) below with your quote. Failure to provide such information with your quote may result in your offer being considered non-eligible. This information may be sent via facsimile at (502) 328-6849. The information or copies required are: (1) A copy of a resume demonstrating your applied knowledge or your training certificate demonstrating capability to provide the specific tasks in C.4.1. through C.4.1.4.; and (2) A certified copy of your health assessment to include the tuberculin skin test examination results. STATEMENT OF WORK -- Computer Lab / Homework Center-Teacher for School-Age Children: C.1. GENERAL: The purpose of the United States Army Yuma Proving Ground (USAYPG) Computer Lab/Homework center for School-Age/Youth Services is to provide a facility for students where they can strengthen their academic and learning skills and increase their opportunity for computer literacy, their employment skills, youth sponsorship and recreation. The computer lab/homework center is one component of the installation's School-Age /Youth Services. A minimum of 30 students per week is anticipated to utilize the computer lab/homework center. C.2. SCOPE OF WORK: The computer lab/ homework center instructor (hereafter referred to as the contractor) shall implement the computer lab/homework center program for School-Age/Youth Services Center children ages 5 to 18 on the installation. C.3. CONTRACTOR: The Contractor shall provide computer lab/homework center assistance and technical guidance to school-age children of military and DOD civilians enrolled in School-Age/ Youth Services on the installation. The contractor shall perform the services set forth in this statement of work (sow). C.4. SPECIFIC TASKS: C.4.1. Computer Lab: C.4.1.1. Contractor must have either applied knowledge of Microsoft Windows, Novell Networks, networking concepts and the Microsoft Windows based applications or is a certified Novell administrator. C.4.1.2. Contractor will provide general software expertise with emphasis on math, science and language (communication) skills. Knowledge of the Internet (i.e., E-mail, World-Wide Web and Gopher) helpful. C.4.1.3. Contractor must be able to troubleshoot problems with the student computer's hardware and software, server hardware and software and other components for the local area network (LAN). The contractor must be able to determine when additional assistance is required to maintain the LAN and be able to identify from whom the assistance should come. C.4.1.4. Contractor will assist students in the completion of daily tasks, reinforce computer skills as required and positively support youth through recognition of their efforts. Contractor will facilitate cooperative learning when appropriate. C.5. HOMEWORK CENTER: C.5.1. Contractor will provide general subject matter expertise with emphasis on math, science and language (communication) skills. Knowledge of computers is helpful. C.5.2. The contractor will assist students in the completion of daily homework assignments, reinforce skills as required and positively support children through recognition of their efforts. Contractor will facilitate cooperative learning when appropriate. C.5.3. The homework center is dependent on the contractor developing linkages with parents, computer lab technician, school teachers, volunteer tutors and others in support of School-Age Services, upon which the successful operation of the homework center is dependent. C.6. COMPUTER LAB/HOMEWORK CENTER: C.6.1. Contractor will be expected to develop linkages with parents, school teachers, volunteer tutors and others in support of School-Age/Youth Services, upon which the successful operation of the computer lab/homework center is dependent. C.6.2. The contractor will be responsible for providing a qualified substitute instructor when unable to instruct. The contractor is responsible for paying all substitutes. Substitutes must be approved by the Contracting Officer Representative (COR). C.6.3. Working Hours: C.6.3.1. Contractor Hours: Total of 33 hours (Monday through Saturday). C.6.3.1.1. Weekdays: The Contractor will work a minimum of five and one-half hours per day Monday through Friday, from 2:30 PM to 8:00 PM for a total of 27.5 hours per week. The Contractor shall work during regular school session and during school inter-sessions. C.6.3.1.2. Weekends: The Contractor shall work the hours 2:30 PM to 8:00 PM on Saturday for a total of 5.5 hours per weekend. The Contractor shall work during regular school session and during school inter-sessions. C.6.3.2. Alternate Hours: In some instances, it may be necessary to alternate the hours set forth in C.6.3 above. In such an instance, the COR shall give notice to the Contractor in writing one-week prior to the scheduled hourly change. The COR notice shall detail the changed hours and the duration of the changed hours. Both the Contractor and the COR shall agree to such a change by both signing the COR request. Copies of any changed hours agreements shall be maintained in the COR files. In any case, the total weekly hours of a changed time period shall not exceed the total number of weekly hours stated within C.6.3.1. above. C.6.4. The contractor will arrive 30 minutes prior to computer lab/homework center opening to allow time for set-up. The contractor will stay 30 minutes beyond the end of the computer lab/homework center session for clean up and/or take down of the environment. Both 30-minute periods are included in the hours stated in paragraph C.6.3.1. above. C.6.5. Vacation/Sick Time: The contractor, in the base performance period of the contract, shall be allowed sixty (60) hours of paid vacation time and twenty (25) hours of paid sick time. In any subsequent option periods the Contractor shall be allowed fifty hours of paid vacation time and twenty hours of paid sick time. All vacation time must be scheduled in advance with the COR and is subject to approval based on workload and mission requirements. If the COR must disapprove leave for any reason, the leave must be rescheduled at a mutually agreeable time. Any time-off in excess of the authorized vacation time or sick time will be deducted at the hourly rate cited in the contract schedule unless an authorized substitute is provided. C.6.7. The Contractor is prohibited from drinking alcohol while performing services and blood alcohol level must be zero (0%) percent while on duty. Contractor is prohibited from smoking while in the presence of youth. C.6.8. The Contractor will work at USAYPG Youth Center, located within Building 1001. C.7. BACKGROUND: C.7.1. The mission of School-Age/Youth Services is to provide supervised out-of-school options that reflects school-age children's interests, reinforce character values and reduce mission conflicts with parental responsibilities. At a recent Department of Defense (DoD) Commander's Conference it was reported that youth (6 to 18) on military installations are experiencing similar challenges as the civilian sector with regards to increased youth violence, gang like activities and juvenile crime. Aside from this likeness, the DoD has identified unique risk factors which impact on the youth of military and DoD civilian families. They include: significant amount of discretionary time; parental deployment absence; lack of supervision; relocation and frequent moves; physical and social isolation; racial and cultural issues. C.7.2. To reduce incidence of at-risk behaviors, the Army has repositioned services for school-age children to significantly increase their supervised out-of-school options. Four main service areas for School-Age Services have been developed for implementation at Army pilot sites. Sports and fitness, Leisure and Recreation, Life Skills and Citizenship, and Monitoring and Support Services. C.7.3. As part of Life Skills and Citizenship, a computer lab/homework center is being established at 44 Army installations. USAYPG has been identified as a pilot site for the Army. The purpose of the computer lab is to provide assistance to school-age children in strengthening their computer learning skills in a safe, age appropriate and supervised setting. C.7.4. This setting will be overseen by the contractor who supports the subject matter areas most critical to the target age group. The results of this pilot effort will have Army-wide impact for development of future School-Age/Youth Services. C.8. SECURITY: The contractor shall be subject to background clearance checks for prior instances of reported misconduct or related misconduct as shown in Table 2-1 of Army Regulation 608-10. Approved substitutes are also subject to these checks. Background checks must be completed prior to working with school-age children. C.9. HEALTH REQUIREMENTS: Contractor will have a medical assessment prior to working with school-age/youth services children. This evaluation will include a tuberculin skin test or chest x-ray. Testing for susceptibility to and vaccination against rubella is strongly recommended. Requirements for immunization against other vaccine preventable diseases will be documented. C.10. CONFIDENTIALITY: Contractor shall maintain strict confidentiality regarding personal information of school-age/youth services children, parents and other staff. C.11. QUALITY CONTROL: Contractor shall establish a quality control program to ensure the requirements of the contract are accomplished as specified. Initial plan shall be presented to the COR within 30 working days of contract award for COR information and approval. Subsequent modifications shall be made as required or to respond to concerns of the COR. At a minimum, quality controls will include the following: C.11.1. A method of obtaining feedback from school-age/youth services children and parents. C.11.2. A means to ensure confidentiality. C.11.3. Provide information regarding significant events and/or concerns that may surface. C.12. PHYSICAL SECURITY: Contractor shall establish and implement methods to ensure the physical security of all U.S. Government property and facilities, while on duty. Methods established shall be in accordance with locally directed physical security and internal control procedures. C.13. SAFETY: Contractor shall be aware of local safety requirements and ensure that all reasonable efforts are made to protect all persons from harm and the U.S. Government property from damage. C.14. GOVERNMENT FURNISHED PROPERTY: The Government will provide to the contractor the following property, for use in connection with this contract: C.14.1. Building -- sufficient space for contractor and school-age/youth services children utilizing the computer lab. C.14.2. Computer lab facility furnished with computers, printers, file server, and all pieces of solid LAN. C.14.3. One class A and one class C telephone available for use within the building. C.14.4. Heat, light, and other utilities. C.14.5. Sharing of office equipment and supplies as needed and available. C.14.6. Standard U.S. Army and Government forms and regulation as applicable to School-Age/Youth Services. C.14.7. Furniture to include tables, chairs, etc. C.14.8. The Government property, made available under this contract, shall be for use only in the performance of the contract. C.14.9. The Government will not be responsible in any way for damage to the contractor's supplies materials, equipment, or personal belongings brought into the building or onto the grounds due to fire, theft, accident, or other disaster. C.15. CONTRACTOR FURNISHED PROPERTY: The contractor shall not be required to furnish any property or materials relevant to this contract, but shall be responsible for receiving, caring, accounting for and returning any Government furnished property while on duty. The contractor should familiarize him/herself with the proper care and use of Government property. C.16. RECORD KEEPING: C.16.1. Contractor shall maintain records on each child with regard to their efforts, accomplishments, and skill areas to be worked on. C.16.2. Contractor shall develop a mechanism for ongoing communication with parents and teachers. C.16.3. Contractor shall maintain program records to track attendance and participation, subject matter needs and linkage within the community. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with your offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition without addenda.
