Description

Provide courier services of laboratory samples, medical documents and packages between the following Community Based Outpatient Clinics (Ellenton, Sarasota, Clearwater, St. Petersburg and Port Charlotte, Florida) and the VA Medical Center (VAMC), Bay Pines, Florida for the period October 1, 2000 through September 30, 2001. Solicitation available on or around September 8, 2000. Only written or faxed requests (FAX 727-398-9536) for the solicitation will be accepted. Bids will be due on or around September 27, 2000. This IFB for commercial items is prepared in accordance with the format in FAR Part 12-Acquisition Commercial Items. FAR Clauses 52.212-1, 52.212-3, 52.212-4 and 52.212-5 will apply. The SIC Code is 4215 and the small business size standard is 18.5 million. See numbered Note 1.
