**CONTINGENT ON FY01 FUNDS! Provide radio maintenance service for 42 ABW & AU radio equipment for FY01. See attachment/s. Attachment #1 STATEMENT OF WORK (SOW) FOR LAND MOBILE RADIO (LMR) MAINTENANCE AT MAXWELL AFB AND GUNTER ANNEX, ALABAMA 1. Scope of Work: This SOW outlines the service necessary to furnish maintenance, maintenance assistance and repair on land mobile radio equipment on attachment #2 for one basic year agreement and two option years selected no later than 15 Aug 2000. Contractor shall furnish all labor, materials, parts, tools and transportation necessary to provide maintenance, maintenance assistance and repair when equipment is determined or considered to be malfunctioning under manufactures specifications and standards. 2. Service by contractor: Provide LMR maintenance 24 hours a day and 365 days a year under normal conditions, military exercises or real world outages due to weather and any other undetermined situation. In addition to response and restoral of equipment, an operational check of the restored asset shall be performed by the user and verified by the 42 Communications Squadron Land Mobile Radio Shop or Maintenance Control Center prior to closing the outage. The following service shall be provided at no additional cost unless otherwise indicated: a. Designate a primary and alternate customer service representative, a 24-hour service call number, fax number and business email address. These representatives shall have full authority to act on behalf of the contractor on all matters related to the daily operations of this contract. Such operations shall include but are not limited to billing, manage contract certifications, provide maintenance and engineering assistance and over the phone troubleshooting. b. Provide technical engineering solutions when less than two hours are spent on any one project. After which time the contractor, prior to proceeding with evaluation services beyond two hours, shall provide an estimate of time and cost. Cost will be paid for through IMPAC rules and regulations. c. Provide a price list of common services available such as mobile radio equipment installation, removal and handheld radio repairs, maintenance and programming. Include non-contracted equipment services available such as public address, cellular phone and antenna equipment installation. List charges associated to after hours work and labor, tech support fees and fees for intermod studies. d. Coordinate with and assist telephone maintenance personnel to restore LMR equipment networks when telephone lines are determined to be at fault. This coordination shall be at prevailing time and material rates paid via IMPAC above one hour from determination and Telephone Maintenance Control or notification to a contract representative below. Normal Duty Hours: Mon -- Fri 7:00 AM to 4:00 PM After Duty Hours: 4:00 PM through 7:00 am, weekends and Holidays (See attachment #4 a list of designated contract representatives) Telephone Maintenance Control Desk Land Mobile Radio Work Center e. The contractor shall be an authorized Motorola Land Mobile Products Service Center and possess a minimum of 3 years experience in radio maintenance management and in commercial Motorola technologies. Only qualified personnel of 3 years experience or more shall service equipment listed under this contract unless supervised by someone meeting the above criteria. The contractor shall submit a valid "Motorola Land Mobile Products Service Certificate" with his or her offer. f. All contract personnel performing maintenance on Data Encryption Standard (DES) radios and associated equipment, must be United States Citizens. FOREIGN NATIONALS ARE NOT ALLOWED TO WORK ON DES EQUIPMENT EVEN IF A US CITIZEN REMOVED THE DES MODULE. Reference: AFI 33-106, paragraph 4.7.11. For access to AFI 33-106, contact Land Mobile Radio at 953-7314. g. The contractor shall submit a valid _Motorola -- Factory Mutual Certification_ to maintain Intrinsically Safe (IS) radio equipment. If the contractor does not have certification, they must have all IS radios repaired by a certifiable depot. h. Authorized representative may direct the contractor to remove, and the contractor shall remove any employee from performance of any service under this contract for reasons of security, unsafe practices or misconduct. When the contractor is notified to remove employee(s) from the premises, replacement shall be at the contractor's expense and not chargeable to the Government. i. The contractor must notify the 42 Communication Sq. Focal Point, (334) 953-7091, if at any time the system being maintained fails to operate for any reason; and advise them the nature of the outage and expected time of repair. j. Perform and document preventative maintenance inspections (PMI_s) on base stations and repeaters at a limit of once a year if requested by authorized government representative. Additional PMI_s will be an additional charge via IMPAC rules and regulations. k. Provide on-site pick-up and delivery for equipment repairs within 8 hours after notification unless, otherwise agreed upon by Land Mobile Radio Managers. Central point of pick up/delivery will be Maxwell AFB, Building 941, room A20, 42 CS/SCMF. l. Initial response: Contractor performing maintenance shall respond to all outages within one hour and provide rapid response for emergency outages under 4 hours and non-emergency outages 8 business hours. m. After initial response, the contractor shall repair or restore radio nets determined by authorized representative. The temporary use of equipment or frequencies is approved for use but will not exceed the average time to repair or replace identical equipment in commercial practices. 3. Billing: Actual charges will be prorated if billing cycle service starts or ends during the month,. All billing shall be charged in the arrears (after service is received) and delivered once a month in electronic format. Forward bills to 42 CS/SCMF (Attn: SSgt King) by use of fax, or electronic mail. If electronic billing is not available, as an interim, one copy of the billing statement will be forwarded to 42 CS/SCMF (Land Mobile Radio). ***Note: "List of Equipment" excel doc. will be provided on request via email.
