Description

The Ralph H. Johnson Department of Veterans Affairs Medical Center intends to award a contract for a primary care internal medicine or family practice physician services. This is considered to be an emergency contract for an approximate 3 month time frame with a 3 month option. The physician must be board certified or board eligible. This is a combined synosis/solicitation for a commercial item service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; and written solicitation will not be issued. The solicitation number is RFP 534-22-01 and is issued as a request for proposal. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-19. this procurement is unrestricted, SIC 8011. Contractor shall provide primary care physician services for the Department of Veterans Affairs, Savannah Primary Care Clinic located at 325 W. Montgomery Crossroads, Savannah, GA 31406 on an hourly basis from approximately October 1-15, 2000 for a 3 month period. It is requested that an option of an additional 3 month period be available. This is to be a firm fixed price contract with an all inclusive hourly rate. Services will be five (5) days per week, 8:00 a.m. unitl 4:30 p.m., Monday through Friday except for Federal Holidays. Please address an overtime rate for any time over 8 hours per day. Curriculum vitae should be included with credentials of the physician that is to perform the services. Payment will be on certified invoices in arrears. Offeror must include a completed copy of the provisions at 52.212-3 Offeror Representations and Certifications -- Commercial Items with it's offer. Original proposals shall be submitted on company letterhead or on Contracting Officer's preferred form (SF1449) to the address above before September 15, 2000 at 2:00 p.m. (local time).
