Description
Background "Description: Design playground, provide and install steel playground equipment with rubberized fall zones at Wright-Patterson Air Force Base, Ohio. PLEASE GO TO www.pixs.wpafb.af.mil TO VIEW COMPLETE SYNOPSIS/SOLICITATION. This is a combined synopsis/solicitation for commercial items as supplemented with additional information included in this notice. This item prepared in accordance with (IAW) the format in FAR Subpart 12.6, announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation F33600-00-T-0771 is Request for Quotations (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-19. The proposed contract being procured is set aside for small business only. The associated standard industrial classification (SIC) code is 3949. The acquisition has one line item (0001). Design playground, provide and install steel, power coated playground equipment, and remove existing equipment at Wright-Patterson Air Force Base, Ohio. Installation shall be completed by 01 Jan 2001. All work will be in accordance with the Specifications, which can be found in the World Wide Web address above. Solicitation provision at FAR 52.212-1 (Mar 2000), Instructions to Offerors -- Commercial are hereby incorporated by reference. The following paragraphs (a) thru (d) are supplemental Instructions (Addendum to Provision 52.212-1) to offerors for development of proposals. (a) Purpose: These instructions prescribe the format of proposal and describe the approach for the development and presentation of proposal data. They are designed to ensure the submission of information essential to the understanding and comprehensive validation of proposals. The instructions permit the inclusion of any additional data or information an offeror deems pertinent. Offerors are cautioned to follow the detailed instructions fully and carefully, as the government intends to make an award based on initial offers received, without discussions of such offers. (b) Proposal Format -- Please return the following: 1) Technical Proposal, 15 page limit (3 Copies) 2) Past Performance Information, 3 page limit (1 copy) 3) Completed Representation and Certifications (52.212-3 attached hereto), 2 pages (1 copy) 4) Completed Price Quote to include schedule and fill-ins (attached hereto), 3 page limit (1 copy) Proposal shall be submitted to: ASC/PKWOS, Area C, Building 1 (Room 109), Attention: Ann Obringer, 1940 Allbrook Drive, Suite 3, Wright-Patterson AFB OH 45433. (c) Technical Proposal: Offerors shall ensure that the technical proposal fully addresses each aspect of the specification. No cost or pricing information shall be included in the technical proposal. Your technical proposal will describe the capability of your organization to provide the requirements of the contract, it should be specific and complete. The proposal shall outline the actual methods proposed if applicable. (d) Past Performance: In their proposal, the offeror shall submit a Present/Past Performance Reference List citing the contract number, name, phone number and fax number of point of contact, dollar value of contract and period of performance on three recent and relevant contracts for the same or similar items. The OFFEROR will submit a copy of the Present/Past Performance Questionaire (attached to this website) to each of the three references with instructions to send completed questionaires to the Contracting Specialist NOT LATER THAN 18 Sep 00. The address label shall reflect the following information: Solicitation Number F33601-00-T0771, ""To be delivered unopened to the Contracting Specialist (A. Obringer)"" and shall be delivered to the proposal address above. Past Performance questionaires may also be faxed to (937)257-3926 (Attn: Ann Obringer). The Government will use information submitted by the offeror as well as other sources to conduct its survey. Solicitation provisions at FAR 52.212-2 Evaluation Commercial Items are hereby incorporated by reference. Evaluation criteria to be included in paragraph (a) of provision 52.212-2 are as follows: 1) Technical 2) Past Performance and Price are equal. Price: Line item 0001 -- Price will be submitted on the entire requirement. The schedule that the offeror may use to submit a price proposal can also be found at the above web location. All offerors shall include with their offers a completed copy of provision 52.212-3, Offeror Representations and Certification Commercial Items. Clause 52.237-1, Site Visit, is hereby incorporated by reference. SITE VISIT: A site visit has been scheduled for 11 Sep 2000 at 9:00 AM EST. Submit the names of all attendees (not to exceed two (2)) no later than close of business 8 Sep 00. Fax number is (937) 257-3926 (ATTN: Ann Obringer.) Include name and SSN of attendees on fax. Attendees should enter Wright-Patterson AFB OH at Gate 12A and proceed to the Visitors Center. A representative will meet attendees and provide transportation to the site. Questions may be faxed prior to the site visit or submitted in writing at the completion of the tour. Meeting minutes and written answers to all questions will be posted on PIXS.
