Description

This is a COMBINED SYNOPSIS/SOLICITATION for commercial items = prepared in accordance with the information in FAR Subpart 12.6 as supplemented = with the additional information included in this notice. This announcement = constitutes the only solicitation; written solicitation will not be issued. PAPER = COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. The RFQ number is = N00140-00-Q-3752 using Simplified Acquisition Procedures under the test program for = commercial items found at FAR 13.5. The solicitation document and incorporated = provisions and clauses are those in effect through Federal Acquisition Circular = 97-11 and DFARS 19990503. It is the contractor's responsibility to be familiar = with the applicable clauses and provisions. The applicable standard = industrial classification code is 8734 and a small business size standard of five = million dollars. FISC Norfolk Detachment Philadelphia intends to award a firm = fixed price contract for the period, effective October 1, 2000 through = September 30, 20001 with three (3) one-year options, to Cytyc Corporation 85 Swanson = Road, Boxborough, MA 01719. FISC Norfolk Detachment Philadelphia intends to = acquire ThinPrep Pap Test Packages for the Naval Hospital Great Lakes, IL: = -CLIN 0001- ThinPrep Pap Test Packages -- 28 PG (P/N 70096-001) 500 GYN Transcyt = Filters (clear); -500 Vials of Preservcyt Solution for ThinPrep Pap Test; 500 = (8), 1/2 Gross Boxes of ThinPrep Microscope Slides; 500 Wallace Papette = Collection Devices. CLIN 0002- Shipping and Handling (1) Lot. Delivery: The = delivery schedule is as follows: CLIN 0001- October (1), November (1), December = (2), January (2), February (2), March (3), April (2), May (3) June = (3), July (3), August (3) and September (3). All items are required to = be delivered on the first Monday of the month.
