Description
DESC: The FISC Norfolk Acquisition Group is issuing a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are hereby requested. A written solicitation will not be issued. This is issued as a Request for Quote (RFQ), Number N00189-00-T-H858 which incorporates provisions and clauses in effect through Federal Acquisition Circular 97-17. This requirement will be 100% Small Business Set Aside. The Standard Industrial Classification Code (SIC) will be 8322, size standard $5 million. This requirement is for one (1) year from 01 October 2000 through 30 September 2001 with four (4) option years. Please provide prices for the following CLIN : of service requested): CLIN 0001, 1 Lot, Director of Religious Education. Statement of Work (FYO1) Director of Religious Education 1.The contractor shall provide professional services for the NSGA Northwest Command Religious Program as Director of Religious Education (DRE) from 00 Oct I- 01 Sept 30. These services will include the formulation and administration of a Protestant and Roman Catholic religious education program for active duty military personnel, retired military personnel, and eligible family members. The contractor is required to discharge the following duties: (a)Coordinate Protestant Sunday School, Children's Church, and Catholic Confraternity of Christian Doctrine (CCD) for authorized personnel. Contractor will provide and maintain a census record for all persons enrolled in Sunday School, Children's Church, and CCD. b)Design, prepare, and coordinate the curriculum calendar of classes and academic/religious events observed throughout the calendar year in concurrence with the Command Chaplain. (c)Prepare all business correspondence and periodic reports on the progress and evaluation of the religious education programs, prepare and direct conferences and special meetings for parents involving religious education instructions. (d) Serve as an ex-officio member of the NSGA Northwest Chapel Council. (e) Maintain files on official church records for all students enrolled in the religious education programs. (f) Interview parents at time of registration regarding the placement of their children as to the proper level of religious education. (g)Prepare and submit an annual budget to the Command Chaplain reflecting the anticipated cost of materials necessary to administer the religious education programs. All materials purchased shall remain in the property of the Command Religious Program and shall be maintained by the contractor. (h)Coordinate and develop a training plan of teachers, assistant teachers, and others who volunteer in the religious education programs. Plans to be submitted to Contracting Officer's Representative (COR) for final approval. (i)Coordinate the Children's Church program. (j)Develop and coordinate a 5-7 day Vacation Bible School during the summer. Contractor will be responsible for selecting the curriculum, ordering material, and recruiting volunteers to teach/substitute. The contractor will be responsible for publicizing this program to NSGA Northwest personnel and housing residents. (k)Shall coordinate all religious education programs and religious education activities for all grade levels throughout the school year, including seasonal celebrations, Confirmation Retreat, an area volunteer program awards. Contractor shall provide for the coordination and planning of transportation arrangements and recruit adequate adult chaperones for all special events. (1) Shall maintain contact with resource personnel and publishing houses to keep abreast of new and vital education resources, materials, and trends. (m) Shall maintain the resource library and audiovisual resources for all religiouseducation programs. (n) Maintain continuous review of all classes in accordance with the plan approved by COR. (o) Make recommendations to COR for utilization and assignment of religious education classroom space for approval by 1 Sep 2001. (p) Contractor shall attend weekly chapel staff meetings and will provide recommendations and scheduling requirements in a timely fashion. 2.The contractor will provide COR a monthly report (IAW standard industry practices) on the last weekday of each month which includes (a) a statistical report of attendance in religious education programs for the reporting month, (b) a listing of teachers and program leaders, (c) a written report regarding the status of future program events, and (d) recommendations, changes or adaptation of the current religious education programs, as required. 3.The contractor shall maintain complete confidentiality of the terms and/or content of this contract instrument and shall discuss any and all issues, first, with the COR, and then with the contracting officer. The contractor shall, at no time, discuss the terms and/or content of this contract with any member attending any of the religious programs. 4.The contractor will perform this contract by being on site at the NSGA Northwest Chapel a minimum of 3 6 hours per week, including three (3) hours on Sunday. Contractor absence (i.e. vacations/holidays) will be arranged two weeks prior to the desired date. Absence due to illness requires immediate telephone notification to COR. Contractor is responsible for providing a listing of possible suitable substitutes in advance of requirements for approval by COR. When absences occur, the contractor will provide and pay a pre-approved suitable substitute to provide religious education services as outlined under this contract. Invoices will be developed by the contractor and submitted to the COR monthly, based on the number of billable hours for the month, for review andapproval. COR will forward invoices to invoice processing office upon approval. 5.Contractor Qualifications (a) Minimum of four (4) years experience (concurrent within last 5- 1 0 years) as a Director of Religious Education or a bachelors degree in Religious Education.Experience with both Protestant and Catholic Religious Education is preferred. (b)Be currently certified in Child CPR and First Aid. (c)Comply with a FBI fingerprint check and State Criminal History Repositor Check (in compliance with 42 U.S.C. para 13041 and DODI 1402.5). (d) Provide a current resume with at least three personal and/or work related references. The government reserves the right to verify qualifications and credentials.
