Description

THIS IS A GOVERNMENT REQUEST FOR INFORMATION. THIS IS NOT A REQUEST FOR PROPOSALS. Veterans Integrated Service Network (VISN) 11 primarily consists of VA Medical Centers located in Ann Arbor, Saginaw, Battle Creek, and Detroit, Michigan, Indianapolis, Marion and Fort Wayne, Indiana, and Danville, Illinois. The VISN 11 network is searching for sources who have the resources and capabilities to establish an on-site, full-service eyeglass laboratory to fabricate and distribute prescription eyeglasses to VA beneficiaries throughout the VISN 11 catchment area. The eyeglass laboratory would be located in space provided by VA at the VA Medical Center, Indianapolis, IN. Distribution of the eyeglasses would be accomplished through the establishment of on-site dispensaries (established separately by the firm through agreements with the Veterans Canteen Service) at larger VA facilities within the VISN and through community-based dispensing sites for smaller VA facilities and in rural areas of the VISN 11 network. It is estimated that in Fiscal Year 2000, VISN 11 facilities will order approximately 13,000 pairs of prescription eyeglasses for VA beneficiaries. All sources that possess the resources, capabilities and interest in this proposed procurement are requested to submit written evidence of such resources and capabilities to the contact point at the above address by the close of business September 25, 2000.
