This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Proposal RFP 518-01-00. This solicitation document provides incorporated clauses that are in effect through Federal Acquisition Circular 97-05. The Bedford VAMC has a requirement for a part time Prosthodontist(3 eight hour days per week between the hours of 8:00am to 4:30pm)to perform a wide variety of dental procedures. Actual days are negotiable. If additional time is required over the hours listed, the amount per hour will be negotiated. Principle duties will be as follows but are not thereto limited to same:(a)Initial and periodic (recall) assessments of the oral and perioral tissues for patients admitted to the ENRM VA Hospital and outpatients as they present themselves for treatment(b)Consultation for other services regarding patient care(c)Invasive and non-invasive diagnostic testing (d)Radiographs, as needed, to facilitate diagnosis and treatment planning(e) Education through its residency program, affiliations and continuing education of staff(f)Oral hygiene preventive dentistry instruction for all patients and care givers(g)Diagnose and treat within the scope of Endodontics(h)Root resection and periapical surgery (apicoectomy(i)Diagnose and treat within the scope of General Dentistry(j)General restorative dentistry for treatment of caries, replacement of teeth(k)Gingival curettage, splinting, occlusal adjustment, root planing(l)Non surgical endodontics, pulp capping, pulpotomy, a root filling(m)Diagnose and treat within the scope of Oral Medicine(n)Dental management of the medically compromised patients(o)Prescribe medications within the scope of Oral Medicine(p)Diagnose and treat within the scope of oral and Maxillofacial Surgery(q)Uncomplicated extractions(r) Surgical extractions(s)Alveolectomy(t)Biopsy (soft tissue,bone)(u)Incision and drainage(v)Root resections(w)Diagnose and treat within the scope of Prosthodontics(x)Overlay dentures(y)Implants(z)Complex fixed and removable prostheses(aa)Research aimed at improving the organization, cost effectiveness, quality and outcomes of oral health care, particularly in the long term care setting(bb)Perform other duties as assigned by the Chief, Dental Service. All responsible sources may submit a written firm fixed price proposal with adequate information to determine professional capability. Contract period will be for One Base Year and Two Option Years with termination notice of 6 months. Evaluations will be made in the following order of importance with price being equal to 50% of total: a. Professional Qualifications b.Availability-proposal will show evidence of sufficient depth to insure coverage of services 52 weeks per year and must show evidence of emergency availability; c.Price Schedule-Schedule should detail cost of normal 8 hour work day and cost of providing services outside of normal 8 hour work day. Proposals must include enough information to adequately evaluate all criteria, incomplete proposals will be rejected. Offers shall provide a complete set of provisions with proposal found at FAR 52.212-3, Offerors, Representation and Certifications-Commercial Items.Provisions and clauses found at FAR 52.212-1,52-212-4,52.212-5 also apply.
