Description
REQUEST FOR INFORMATION, SOLICITATION DADA10-00-R-0056. POC MIKE GONZALES 210-295-4357 OR LEO M SLEIGHT 210-295-4357 (SITE CODE DADA10). The U S Army Medical Command, MEDCOM Contracting Center, is soliciting information from businesses regardless of size at assist us in preparing a Statement of Work (SOW)to contract for two, possibly three, primary care clinics. One is to be located in the Fairfax/Burke, VA area and the other in the Woodbridge/Dale City VA area. The third clinic location will be decided on at a later date. Fairfax Clinic zip codes are 22002, 22003, 22031, 22015, and 22151. Woodbridge Clinic zip codes are 22192 and 22193. The contracting services are medical, clinical, and administrative staff support to an ambulatory primary care program as defined in Army Regulation AR 40-3 for the treatment of authorized military beneficiaries with acute and chronic care conditions. The clinics are designated as primary care entry points with an enrolled population of approximately 58,000 beneficiaries of the DeWitt Health Care System and will offer mental health services, optometry services, physical therapy, orthopedics, OB, allergy, dermatology, ENT, and nutritional care. The Family Health Center of Woodbridge is expected to enroll up to 35,000 beneficiaries. The Family Health of Fairfax is expected to enroll up to 23,000 beneficiaries. Additionally, some limited space available care for non-enrolled beneficiaries may be offered. This is a non-personal services contract covering a diversity of professional disciplines. This contract will provide primary and specialty health care to members of the Uniformed Services and their families, and other authorized beneficiaries of the Department of Defense's (DOD)regional managed care program, TRICARE, and the DACh Primary Health Care System. The clinics will be opeated by the Commander, Fort Belvoir U S Army Medical Department activity. The Hospital Commander or designated representative will provide medical management and oversight. The clinics must meet the medical quality assurance standards in the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), Ambulatory Healthcare Standards Manual, and applicable U S Army Regulations. Contractors must demonstrate capability to provide supplemental professional and ancillary staff to operate two primary care clinics, provide ambulatory family practice services for acute and chronic care conditions. Responses are due this office in writing not later than 18 Sep 2000 and shall not exceed more than 2 pages. Quality control program and financial information may be requested when the formal request for proposal is issued.
