Description

Single award term contract for the Department of Health and Human Services, Center for Disease Control, Atlanta, GA, for the term of December 1, 2000 through November 30, 2001, and one option year. Quality Level III is required. These specifications cover the production of 2 and 3 part scan form sets requring such operations as typesetting, film-making, printing, serial numbering with sequential numbers, pre-slugging of numbering (part 1 only), attaching six piggy-back style labels (with corresponding numbers) to each form, perforating, double stubs 11" way with firm glue, shrink wrapping, packing and distribution. Part 1 of each set is the only scannable sheet (parts 2 and 3 are duplicates only) and is to be processed on a NCS Sentry 3000 Mark Sense Reader and OPSCAN 5, and must be guaranteed to function on same. Approximately 4 orders per year. 250,000 to 500,000 copies per order. Films required on all orders. Deliver f.o.b. destination to CDC Warehouse, Chamblee, GA. Complete production and delivery must be made within 25 workdays for the first 200,000 sets. An additional 4 workdays will be allowed for each additional 50,000 sets, not to exceed a total of 35 workdays for any order.
