Description

Create negative for (1) survey questionaire from e-mail version, where information has been stored in the Design Expert software. Overnight copy of color proof with in 2-4 days of receipt of e-mail to Millington, TN 38055. Upon approval, print survey instrument using offset lithography. Printed survey must be readable by an OPSCAN 10 Model 55 pencil read head. 155,000 eight page booklets (two -- 11 x 17 signatures) 2 color forms perforated and with liho-codes and serial numbers on front page must be printed and shipped withing 20 working days of receipt of approval. Paper requirements: 100% wood pulp paper without water marks, embossed or printed patterns, or fluorescent additives. Weight: 60 to 80 lb. Color: white. Must be resistant to curl, and free of foreign elements causing false mark detection or failure to detect light valid marks. Caliper: .004" -- >008", Smoothness (sheffeild): 100-400, Dimension stability with an 8 1/2" length, Grain direction: plus 0.010", minus 0.005", Cross direction: plus 0.025", minus 0.010", Reflectance: min 70%.
