Description

This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is F41622-00-R-0029 and is issued as a request for quotation (RFQ). This solicitation document and incorporated provision and clauses are those in effect through Federal Acquisition Circular 97-13. The Contractor shall provide Biomedical Equipment Maintenance Services at the Great Lakes Naval Training Center. Contractor shall furnish all labor, management, supervision, consultations and reports. Contractor services shall cover the range of service provided in a civilian medical treatment facility. Performance shall be according to the requirements contained in the performance work statement (PWS). For a copy of the PWS please e-mail Capt Bentle at jenise.bentle@brooks.af.mil. Period of Performance is one year from date of award (estimated award date: 29 Sep 00). The provisions at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. Offerors must include the Representations and Certifications found in FAR 52.212-3. The clause at FAR 52.212-4 applies. The clause at FAR 52.214-5 applies with the inclusion of (b)(6)(7)(8)(9)(10) and (c)(1). Bids are due no later than close of business 15 Sep 00. POC is Capt Jenise A. Bentle at (210)536-2165.
