Description

The U.S. Naval Hospital, Guam has a requirement to purchase the following: 1 ea Amsco 3085 SP Surgical Table, Catalog #BL5473053000000000, and 3 ea Amsco Uni-Matic II Scrub Station. This proposed contract action is for commercial items, prepared in accordance with the format in FAR subpart 12.6, and incorporates the following FAR clauses and provisions: 52.212-1, 52.212-2, 52.212-4 and 52.212-5. All offerors shall include with their offers a completed copy of provision 52.212-3, Offeror Representations and Certifications-Commercial Items, and addendum clause DFARS 252.225-7035, Buy American Act-Balance of Payments Program Certificate. Important Notice: DFARS 252.204-7004, "Required Central Contractor Registration" applies to all solicitations issued on or after 01 June 1998. Lack of registration in the CCR database will make an offeror/quoter INELIGIBLE FOR AWARD. This acquisition will be processed under Simplified Acquisition Procedures (SAP)and is unrestricted.
