Description

The Department of Veterans Affairs, John D. Dingell Medical Center is seeking request for quotes to provide preventive maintenance (parts and labor) and operation verification procedures on 14 Bear Medical System Ventilators, Model Number 1000. This is a combined synopsis/solicitation for a commercial item, prepared in accordance with the subparts in Federal Acquisition Regulation 12.6 as supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through Fderal Acquisition Circular 97-13. Provisions and clauses found at 52.212-1, 52.212-4, 52.212-5. The Standard Industrial Classification Code is 7629, Electrical Repair shops (Medical). The very small business size standard is 15 or less employees. Offerors may find the FAR provisions on the internet at www.arnet.gov/far. Proposed rates should include all costs associated with providing the following services. The accurate performance of these duties not only has an impact on the quality of patient care, but also the effectiveness and efficiency with which care can be provided. All responses must include a list of replacement items that are normally used when performing preventive maintenance. Contractor will be required to perform preventive maintenance within 10 working days of notification from Biomedical Engineering. Contractor will be required to attend Safety Training as required by the Safety Office of the VA Medical Center. This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. Solicitation Number Request (RFQ) 553-103-00 is assigned and shall be referenced on any proposals. Partial proposals shall be rejected. All potential offerors shall be available to perform the requirements of this contract immediately. Offerors shall provide a complete set of provisions with proposal found at FAR 52.212-3, Offeror Representation and Certifications-Commerical Items. Offerors must also submit with their bids written evidence of training and proficiency (statement of competency)in repairing or maintaining the equipment on which they are working. Proposals shall be submitted to the address listed herein or faxed to 313-576-1077 and directed to the attention of Marilyn Dogan (001R-A). The government shall not be held responsible for quality of fax transmittals. The solicitation number (RFQ number) shall be listed on the outside of your response for identification.
