Description
POTENTIAL SOURCES SOUGHT FOR RFP: The Department of Veterans Affairs Medical Center, Long Beach Healthcare System, Long Beach, CA (VALBHCS) has a requirement for a Capitated Community Based Outpatient Clinic (CBOC) for continuous primary care services for VA enrolled veterans in the Whittier, CA or surrounding communities of Whittier in the Los Angeles County area. The Offeror (s) place of performance/facility to provide services must be within a ten (5) -- twenty-five (25) mile radius outside Whittier, CA corporate city limits or physically located with the city limits of Whittier, CA. The intent is to provide primary care services by a local provide within the community. The offeror will provide primary care services through scheduled visits. Scheduled visits may be Monday through Saturday but no earlier than 7:00 a.m. and no later than 8:00 p.m. Clinic Hours of Availability will be established prior to contract award. The VA Long Beach Healthcare System intends to issue a solicitation for the purpose of contracting with a "full service/all inclusive" capitated firm-fixed price continuous primary care services with an offeror that can provide continuous primary care services exclusively to veterans only at the hours or scheduled clinic days agreed upon by both parties prior to contract award. The clinic shall provide 20 hours of primary care services exclusively to veterans only with availability to expand the number of hours for primary care services to veterans only. Veterans enrolled in the clinic and their eligibility will be determined by VALBHCS. Services reimbursement rate will be at a "full service/all inclusive" capitated rate (per member, per month) rate in accordance with the terms and conditions of the contract. The contract will include a base year, with four (4) One-year renewal options that can be exercised at the option of the Government. All hospitals, medical centers, outpatient clinics, physician practices (either family practitioners or general internist), physician groups, hospital management groups, preferred provider organization, health maintenance organization groups and fee for service groups, etc. are encouraged to participate. The offeror shall provide health care clinical providers, medical facilities, medical facilities, medical equipment and supplies, pharmacy services (in accordance to specified contract terms) and all administrative and management functions of all aspects of primary care services to fulfill the level of care and range of services required by the contract terms and conditions. Primary care services to the veterans will be continuous for the duration of the contract term(s).
