Description

VA Medical Center, Memphis, TN intends to issue a solicitation for Community Based Primary Care Services. The intent is to provide primary care services by a local provider within the community. Veterans will be enrolled in the clinic by the Department of Veteran Affairs Medical Center, Memphis, TN for the period of one year from Notice to Proceed. Veterans to be serviced under this capitation payment contract reside primarily in Hardin County, Tennessee and the surrounding counties. The Health care Provider must physically be located within the city limits of Savannah, TN or no more than 5 miles outside the corporate city limits of Savannah, TN. This contract will include a base year, with two, one-year renewal options that can be exercised at the option of the Government. Closing date is October 13, 2000 4:30 PM local time.
