Description

(i) This is a combined synopsis/solicitation prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This synopsis/solicitation is for aerobics instructors for fitness programs in the Scott Air Force Base Fitness Center for a performance period from 1 Oct 00 to 30 Sep 01. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) The reference number is F7345501440100 and the solicitation is issued as a Request For Quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 97-19. (iv) This is a total small business set-aside. The associated standard industrial classification code is 7991 and the small business size standard is $5.0 million. (v) The solicitation can be viewed at http://www.eps.gov/. (vi) The solicitation package includes the Statement of Work (SOW), Wage Determination 94-2309 Rev (19) dated 07/20/2000, and provisions/clauses. (vii) The provision at Federal Acquisition Regulation (FAR) 52.212-1, Instructions to Offerors- Commercial, applies to this acquisition. (viii) The FAR provision 52-212-2, Evaluation- Commercial Items, applies to this acquisition. Paragraph (a) is completed as follows: Price and Past Performance. Price is significantly more important than Past Performance when being evaluated. Past Performance will be evaluated but not rated. (ix) A completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, must be submitted with the quote. The provision may be copied from website http://farsite.hill.af.mil/reghtml/far/52_000.htm. Quotes will not be considered without the completed provision. (x) The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. (xi) The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. The clauses at subparagraph (b)(5), (b)(12), (b)(13), (b)(14), (b)(15), (b)(22), (c)(1), and (c)(2) of 52.212-5 are applicable. (xii) The clause at FAR 52.232-18, Availability of Funds, applies to this acquisition. (xiii) Defense Federal Acquisition Regulation (DFAR) clause 252.204-7004, Required Central Contractor Registration, applies. (xiv) All responsible sources may submit a quotation, which shall be considered by the Agency. (xv) Contract award cannot be made to any contractor who is not enrolled in the Department of Defense (DoD) Central Contractor Registration (CCR) program.
